中国5~14岁儿童呼气峰流速正常值及预测值方程式的建立

目的建立中国5~14岁儿童呼气峰值流速(PEFR)正常值范围和预测值方程式。方法根据中国地理分布选取5个研究中心:华南(广州)、华东(苏州)、西南(成都)、西北(西安)、华北(北京),随机抽取2010年10月~12月就读于各研究中心的学生发放问卷和知情同意书。其中问卷填写正确、家长签署知情同意书、年龄在5~14岁、无任何心肺及其他系统疾病史、近4周无呼吸道疾病史,当面询问和体格检查呼吸、心率、胸廓、肺部体及身高、体质量均在正常范围者作为研究对象。本研究经医院伦理委员会同意并备案。采用英国制造的Vitrograph简易峰流速仪,于上午8:00-12:00采取坐姿行峰流速测定6次,记录3次最好值。以实测指标采用SPSS 20.0统计软件,经多元逐步回归和多元线性回归,得出预测值方程式。结果共纳入符合研究对象标准的儿童3169名,男1596名、女1573名;其中各中心的纳入研究对象、男、女人数分别为:北京(365,184,181)、成都(423,201,222)、苏州(1046,517,526)、西安(942,481,461)广州(396,213,183),每个年龄段人数均等。PEFR值随着年龄的增长,统计有差异,P<0.001;男孩比女孩高,其中5岁,10岁,11岁组的男女性简易PEF值差别没有统计学意义,其他年龄段差别均有统计学意义,(P<0.05)。PEFR与年龄、身高、体重、BMI都具有相关性,其中PEF值与身高的相关性最强。结论本研究建立了中国5~14岁儿童呼气峰值流速(PEFR)正常值范围和预测值方程式。PEFR值随年龄增高而增高,其受儿童身高影响超过体质量和年龄变化。 Objective To establish the normal range and prediction equation of peak expiratory flow (PEFR) in Chinese children aged 5-14 years. Methods According to the geographical distribution in China, five research centers were chosen: South China (Guangzhou), East China (Suzhou), Southwest (Chengdu), Northwest (Xi’an) and North China (Beijing) Of students issued questionnaires and informed consent forms.
