Background: In Taiwan, suicide has been the ninth-leading cause of death since 1999.Although QPR (question, persuade, and refer), the golden model of gatekeeper for suicide prevention in the world, has been conducted and promoted by Taiwan Suicide Prevention Centre since 2005, the effectiveness of suicide prevention was limited.It might because many people refuse to be referred and worry about being labelled.Moreover, the reference system is not sufficient and complete in Taiwan.As a result, a new gatekeeper model, BHC-5L (being happiness catchers-looking, listening, learning, loving, living) has been developed for Eastern culture by New Taipei City Government, Chung Shan Medical University, and Mackay Memorial Hospital.The purpose of the study was to evaluate the effectiveness of this new gatekeeper training program.Methods: The gatekeeper training program was conducted from July to August 2010.Participants should be trained by the two-day course and complete both the pre-test and post-test questionnaires.Among 72 participants, 62 people accomplished the training program and became qualified gatekeepers.Paired t test and proportional odds model were used to analyze the effectiveness of the gatekeeper training program.Results: After the training, participants reported great understanding of the meaning of BHC-5L (mean score =17.87, the total score ranged from 5 to 20).In addition, they indicated that the BHC-5L lectures did significantly improve their skills of suicide prevention and also increase their confidence to be a trainer (mean of difference =0.66; t =4.29; p<0.001).Furthermore, they felt the BHC-5L lectures did significantly increase the gatekeepers competence to help someone with suicide ideation or attempt, and also survivors (mean of difference =0.29, t =2.12, p =0.03).After adjusting the gatekeepers sex, age, and experience, if they realize the lectures more than others, they will have more confidence to be a trainer (OR =1.61; 95%C.I.=1.23, 2.10; p < 0.001) and help someone with suicide ideation or attempt, and also survivors (OR =1.55; 95%C.I.=1.19, 2.01; p < 0.001).Conclusion: The two-day lectures of BHC-5L offered the necessary knowledge and skills of suicide prevention to all gatekeepers, and also increased their confidence.It might help us to create a new gatekeeper model in Eastern culture.
