South Africa carries an incredible burden of HIV:more than 5.5 million people are infected,more than 30% of all pregnant women are HIV-seropositive and about 250000 individuals take highly active antiretroviral therapy (HAART).The National Comprehensive HIV/AIDS Treatment Plan was based largely on the experience gained at the Khayalitsha and the Lusikisiki MSF supported treatment sites.In resource-limited rural settings,scaling up ofantiretroviral treatment (ART) is usually not met by a proportional scale up of health care workers.Appropriate models of service delivery thus needed to be defined for rural areas to ensure an effective and equitable HIV service.
