According to a recent meta-analysis, diabetic retinopathy has a prevalence of roughly one third among patients with diabetes mellitus; this prevalence may be lower among Asians than among Caucasians; factors associated with the prevalence of diabetic retinopathy were known duration of diabetes mellitus, quality of metabolic control (HbA1c value), blood pressure, and type 1 versus type 2 of diabetes mellitus.Current therapeutic modalities include focal or panretinal laser coagulation, leading to irreversible retinal tissue destruction, and / or intravitreal medical therapy including crystalline cortisone (triamcinolone acetonide), slow re-lease devices loaded with steroids (dexamethasone, fluocinolone), and specific anti-VEGF drugs such as bevacizumab, ranibizumab and aflibercept.Future therapeutic directions may be the intravitreal application of mesenchymal stem cells to further support the existing retinal cells, stem cells to replace retinal cells lost in the diabetic process, intravitreal cell-based drug production, and measures to influence the retinal microglial cells as gardeners and garbage cells of the retina.
