Inferior platelet inhibition post percutaneous coronary intervention (PCI) is related to higher cardiovascular events.However, by doubling olopidogrel doses in patients with inferior platelet response is controversial.We used VerifyNow P2Y1 2 platelet inhibition test on high risk patients (in-stent thrombosis, left main disease, left arterial descending disease, bifurcation lesions or significant drug interactions).Based on the test results, we choose an antiplatelet agent (clopidogrel vs prasugrel) at an appropriate dose to achieve 40% or higher platelet inhibition.We are able to minimize composite cardiovascular events.We conclude, in patients with in-stent thrombosis, left main disease, left arterial descending disease, bifurcation lesions or unavoidable significant drug interactions, P2Y12 platelet inhibition test can be helpful to maximize the efficacy of antiplatelet treatment in patient post PCI.
