目的 检测血清肿瘤标志物AFP、Paternally expressed gene 10(PEG10)、细胞角蛋白19(CK19)在原发性肝癌、肝硬化和健康人血清中的水平，探讨其单独检测和联合检测对原发性肝癌的临床诊断价值，分析CK19、PEG10对AFP阴性原发性肝癌的诊断价值.方法 收集河南省人民医院2012年12月至2013年11月期间住院患者的血清标本156份，患者年龄为32～81岁，平均(58.0±11.5)岁.包括原发性肝癌96例，健康人30名，肝硬化30例.ELISA法检测各组血清AFP、CK19、PEG10水平，组间比较采用方差分析.AFP、CK19、PEG10诊断原发性肝癌的最佳临界值，采用ROC.结果 血清AFP、CK19、PEG10在原发性肝癌组显著高于肝硬化组和健康组，差异有统计学意义(P＜0.05).AFP、CK19、PEG10诊断原发性肝癌的敏感度分别为85.4％、42.9％和72.7％.特异度分别为80.0％、87.2％和86.9％.AFP与CK19、PEG10分别行平行联合检测，可提高原发性肝癌诊断的敏感度；AFP与CK19、PEG10分别行系列联合检测，可提高原发性肝癌诊断的特异度.CK19在AFP阴性原发性肝癌组中的水平明显高于肝硬化组，差异有统计学意义(P＜0.05)；PEG10在AFP阴性的原发性肝癌组与肝硬化组差异无统计学意义(P＞0.05).CK19对AFP阴性的原发性肝癌诊断的准确度高，敏感度为50.0％，特异度为90.9％.结论 CK19、PEG10与AFP联合检测可提高原发性肝癌诊断的敏感度和特异度.CK19对AFP阴性的原发性肝癌诊断的准确度最高，在原发性肝癌的早期诊断中可能作为AFP有价值的补充.
