Introduction: Hip fractures are common, especially in the elderly patients.As the elderly population grows, the number of hip fractures continues to increase. A total of 310,000 individuals were hospitalized with hip fractures in the United States alone in 2003, according to data from the United States Agency for Healthcare Research and Quality (AHRQ), accounting for 30 percent of all hospitalized patients.Worldwide, the total number of hip fractures is expected to surpass 6 million by the year 2050. Moreover, the cost of hip fractures treatments is expensive, treating hip fractures has become a major health care problem. China is a large developing country facing populationaging problem, the economic conditions and medical conditions are highly different among regions. The incidence of hip fractures is high in China, but there is little multicenter study on the cost of hip fractures hospitalization in China. Objective: The aim of this study was to compare the hospitalization cost of hip fractures patients in different provinces of Chinawiththe GDP per capitaand average annual disposable income, and to provide rational proposals for national medical insurance policy. Material and Methods: Wesearched the Chinese National Medical Data Center Database and selected all the patients with the main diagnose offemoral neck or intertrochanteric fracture. Their hospitalization costs, treating methods and hospitalization provinces were gathered and analyzed using SPSS 23.0. The average annual salary and average annual disposable income were obtained from the website of National Bureau of Statistics of the Peoples Republic of China. Results:31168 cases of 73 central hospitals from 24 provinces were included in the study; their treatments were mainly divided into three kinds: hip replacement, open reduction internal fixation and conservative treatments.The distribution of hospitalization cost was described under different variable factors of provinces,treatmentsmethods and diagnoses. The average cost of all patients was 48175 RMB, which wasmore than the GDP per capita of 2014 (46629 RMB) and was 2.38 times of the average annual disposable income of 2014 (20167 RMB). On average, femoral neck fracture patients spent 48176 RMB, and intertrochantic fracture patients spent 49314 RMB. Patients receiving conservative treatments spent 47173 RMB, those receiving internal fixation spent 46089 RMB, and those receiving hip replacements spent 61905 RMB on average. The hospitalization cost of hip fracture has become a great burden to the patients families, but the medical insurance nowonly covers 70% of the hospitalization costfor patients receiving internal fixation, and only 15% for those receiving hip replacement. The medical insurance coverage percentage is even lower in some provinces. The medical insurance policy is still irrational and need to be revised.
