Objective：To discuss early experience of the management for acetabular defects in total hip arthroplasty in 31cases . Methods 31cases acetabular defects in total hip arthroplasty were retrospectively analyzed.There were 5 segmental acetabular defect cases, 7 cavity acetabular defect cases ,6 acetubular fracture and defect cases , 8 fusion hip cases. The new total hip arthroplasty were applied in all 31 cases. Results：The operations were succeed in 31 cases wtihout any infection. The mean time of fellow-up was 24.3 months with a range of 6～68months. The function was good. Conclusions：(1) The good result was come from the application of morselized bone only or supplement with structure bone autografting. It was better to fixed structure bone by the plate and screw system. (2) The pedicle sartorius muscle iliac bone grafting was the first application for acetabular defects in total hip arthroplasty in 2 cases. The new bone formation was better.(3) Uncemented acetbular implant-cup gives superior outcome compared to cemented one.
