Objective: Postoperative hemorrhage is one of the most fatal complications after abdominal surgery.Transcatheter arterial angiography and embolization has been used for the treatment of postoperative hemorrhage after abdominal surgery.Methods: This retrospective study included twenty-six patients with postoperative hemorrhage after abdominal surgery.All patients underwent emergency transarterial angiography,and seventeen patients underwent emergency embolization.We retrospectively analyzed the angiographic features and the clinical outcomes of transcatheter arterial embolization.Results: Angiography showed that a discrete bleeding focus was detected in 21 (81%) of 26 patients.Three of five patients without positive angiographic findings in our study received second surgery.And the bleeding vessels were come from splenic vein,portal vein behind the gastrointestinal anastomotic stoma,and left gastroomental vein,respectively.Positive angiographic findings included extravasations of contrast medium (n=9),pseudoaneurysms (n=9),and fusiform aneurysms (n=3).Transarterial embolization was technically successful in 17 (100%) of 17 patients.Clinical success was achieved in 14 (82%) of 17 patients.No postembolization complications were observed.Three patients died of rebleeding.Conclusion: Angiography is sensitive to detect the bleeding site in patients with postoperative hemorrhage after abdominal surgery.It is difficult for angiography to find the bleeding site come from veins.Transcatheter arterial embolization is an effective and safe method in the management of postoperative gastrointestinal hemorrhage.
