目的探讨螺旋CT三维重建技术在儿童先天性髋关节脱位(CDH)手术前的应用价值。方法对先天性髋关节脱位患者45例(59髋),行双侧髋关节螺旋CT三维重建检查,测量前倾角(FNA)、髋臼指数(AI)及髋臼壁的发育情况,与术中所见比较。结果45例患儿前倾角均加大,最大90°,最小40°,平均42.72°±12.06°,髋臼指数均有不同程度加大,平均35.4°±9.5°,髋臼各壁不同程度发育不良,与术中所见符合。结论髋关节螺旋CT三维重建技术可以直观、全面及分解地显示髋关节的病理形态及发育不良的程度,更加精确地测量AI和FNA,提供了一个更具真实感的三维立体影像,对术者选择适当的治疗方案及手术入路,纠正前倾角及髋臼指数均具有重要的指导意义。
Objective To investigate the value of spiral CT three-dimensional reconstruction in children with congenital dislocation of the hip (CDH). Methods 45 patients (59 hips) with congenital dislocation of the hip were examined by three-dimensional reconstruction of bilateral hip helical CT. The anteversion (FNA), acetabular index (AI) and development of acetabular wall were measured. See comparison. Conclusion Three-dimensional reconstruction of hip helical CT can intuitively, comprehensively and decomposely display the pathological morphology and degree of dysplasia of the hip joint. It can measure AI and FNA more accurately and provide a more realistic three-dimensional image. Select the appropriate treatment options and surgical approach, correction of anteversion and acetabular index has an important guiding significance.
