对糖尿病合并非酒精性脂肪肝患者进行血脂、空腹血游离脂肪酸(FFA)、空腹血糖(FPG)及血浆胰岛素(FINS)的测定,计算胰岛素抵抗指数(HOMA -IR) ,并与单纯肥胖非脂肪肝糖尿病患者比较。结果:脂肪肝组与非脂肪肝组比较,甘油三酯(TG)、空腹血游离脂肪酸(FFA)、血浆胰岛素(FINS)水平、胰岛素抵抗指数(HOMA -IR)明显升高(P <0 .0 5 ) ,高密度脂蛋白(HDL)水平明显降低(P <0. 0 5 )。结论:糖尿病合并脂肪肝患者存在着明显的胰岛素抵抗及脂质代谢异常,与非脂肪肝患者比较有显著性差异,提示胰岛素抵抗与脂质代谢异常在脂肪肝的发病中具有十分重要的地位。
The levels of serum lipids, free fatty acids (FFA), fasting plasma glucose (FPG) and plasma insulin (FINS) in diabetic patients with non-alcoholic fatty liver were measured. HOMA-IR Liver Diabetes Comparison. Results: The levels of triglyceride (TG), fasting blood free fatty acid (FFA), plasma insulin (FINS) and insulin resistance index (HOMA-IR) in fatty liver group were significantly higher than those in non-fatty liver group (P < 0 5), and the level of high density lipoprotein (HDL) decreased significantly (P <0.05). CONCLUSION: There are obvious insulin resistance and lipid metabolism abnormalities in patients with diabetes mellitus complicated with fatty liver, which are significantly different from non-fatty liver patients, suggesting that insulin resistance and abnormal lipid metabolism play an important role in the pathogenesis of fatty liver.
