目的探讨肺部肿瘤病房使用新模式,通过增加床位周转,缓解目前肺部肿瘤患者住院难的问题,同时在保证患者疗效情况下进一步降低患者的医疗费用,节省医疗开支。方法统计上海交通大学附属胸科医院肺部肿瘤医学中心2009年1月—2009年4月接受日间化疗患者(n=249)的治疗费用、住院时间及化疗相关不良反应的发生情况,以同期接受普通化疗的患者(n=827)作为对照组。结果日间化疗组患者的平均住院时间和治疗总费用均显著低于与普通化疗组,差异具有统计学意义(4.30 d和13.7 d,6 751.87元和14 198.03元,均P<0.01)。两组化疗相关不良反应相似,日间化疗组白细胞降低比例高于普通化疗组,胃肠道反应(呕吐)发生比例低于普通化疗组,差异均具有统计学意义(P<0.01)。结论日间化疗有效缩短了患者的住院时间,减少了住院费用,可作为常规住院化疗外的治疗新模式进一步推广。
OBJECTIVE: To explore the new model of lung cancer ward, which can relieve the current difficulty in hospitalization of patients with lung cancer by increasing the turnover of beds, and at the same time, further reduce the medical expenses of patients and save the medical expenses while ensuring the curative effect of patients. Methods Statistical analysis was performed on the cost of treatment, length of stay and the incidence of chemotherapy-related adverse events among patients who received daytime chemotherapy (n = 249) from January 2009 to April 2009 in the Department of Lung Cancer Medical Center, Shanghai Chestnut Hospital, Shanghai Jiao Tong University. Patients receiving normal chemotherapy (n = 827) served as controls. Results The average length of hospital stay and total cost of chemotherapy in the chemotherapy group were significantly lower than those in the normal chemotherapy group (4.30 d and 13.7 d, 6 751.87 and 14 198.03 respectively, both P <0.01). The two groups of chemotherapy-related adverse reactions were similar in the daytime chemotherapy group, the proportion of leukopenia was lower than that in the normal chemotherapy group, and the incidence of gastrointestinal reaction (vomiting) was lower than that in the normal chemotherapy group (all P <0.01). Conclusion Daytime chemotherapy effectively shortens the hospitalization time and reduces the cost of hospitalization, which can be further promoted as a new treatment mode outside of routine hospitalization chemotherapy.
