目的探讨应用不合理处方联络单对提升门诊药房药学服务水平、减少用药差错、保障患者用药安全的作用。方法对漳州正兴医院行不合理处方联络单制度前后,即2013年9月—2014年2月与2014年3月—2014年8月门诊药房不合理处方发生率进行统计分析。结果 2013年9月—2014年2月(实施不合理处方联络单制度前)不合理处方数为306张,2014年3月—2014年8月(实施不合理处方联络单制度后)不合理处方数为102张,在102张不合理处方中,医师采纳建议94张,采纳率为92.16%。门诊不合理处方主要表现在处方用药与临床诊断不相符,给药频次、用量、用法不正确,抗菌药物使用指征不明确等方面。结论应用不合理处方联络单可提升门诊药房药学服务水平、减少用药差错、保障患者用药安全。
Objective To explore the application of unreasonable prescriptions to improve the pharmacy service level of outpatient pharmacy, reduce medication errors and ensure the safety of patients. Methods The incidence of unreasonable prescriptions in outpatient pharmacy before and after the unreasonable prescription contact system was investigated in Zhangzhou Zhengxing Hospital from September 2013 to February 2014 and from March 2014 to August 2014 was statistically analyzed. Results From September 2013 to February 2014 (before implementation of the system of unreasonable prescription liaison), the number of unreasonable prescriptions was 306, and from March 2014 to August 2014 (after the implementation of the system of unreasonable prescription liaison) unreasonable prescription The number of 102, in 102 unreasonable prescriptions, the doctor adopted 94 recommendations, the adoption rate was 92.16%. The out-patient unreasonable prescription is mainly manifested in prescription drug and clinical diagnosis is not consistent, frequency of administration, dosage, usage is not correct, the indication of the use of antibacterial drugs is not clear and so on. Conclusion The application of unreasonable prescription liaison list can improve the pharmacy service level of outpatient pharmacy, reduce medication errors and ensure the safety of patients.
