目的了解免疫受损大鼠铜绿假单胞菌肺部感染前后肺组织中细胞间黏附分子(ICAM)-1、基质金属蛋白酶(MMP)-2和MMP-9的改变及其与肺部炎症反应和肺损伤的关系。方法建立免疫受损大鼠铜绿假单胞菌肺部感染的模型,观察其肺部炎症反应,测定湿/干比和支气管肺泡灌洗液中总蛋白(TP)了解肺损伤情况,对其肺组织分别行ICAM-1、MMP-2和MMP-9免疫组织化学研究。结果1.铜绿假单胞菌肺部感染后,两组大鼠肺泡上皮细胞中ICAM-1染色强度较未感染时增加,接种后6h、9h、24h明显高于接种前(P<0.05);2.两组大鼠在铜绿假单胞菌肺部感染后,肺组织中MMP-2染色增强,于接种后6h或9h达高峰,并持续至24h。两组大鼠感染前后MMP-2在各组织结构中的积分差异无统计学意义。MMP-9在感染前后的改变与MMP-2相仿; 3.两组大鼠肺组织中ICAM-1积分与中性粒细胞半定量计数具有相关性(对照组r_s=0.641,P= 0.001,免疫受损组r_s=0.668,P<0.001);4.两组大鼠肺组织中MMP-2、MMP-9染色积分与PMN半定量计数呈正相关(P<0.01);5.免疫受损组大鼠支气管上皮细胞中MMP-9染色强度积分与TP具相关性(r_s=0.484,P<0.05),肺泡上皮细胞、小血管内皮细胞及内皮下组织中MMP-2染色强度与TP皆具相关性(r_s分别为0.457、0.492、0.429,P<0.05)。结论免疫受损大鼠肺部铜绿假单胞菌感染后,肺组织中ICAM-1、MMP-2、MMP-9免疫组织化学显示染色增强,于感染后6~9h达高峰,与免疫受损大鼠肺组织中明显的中性粒细胞浸润及严重的肺损伤有关。
Objective To investigate the changes of intercellular adhesion molecule (ICAM) -1, matrix metalloproteinase (MMP) -2 and MMP-9 in lung tissues of immunocompromised rats before and after Pseudomonas aeruginosa pulmonary infection and their relationship with lung inflammation And lung injury. Methods To establish a lung infection model of Pseudomonas aeruginosa in immunocompromised rats and to observe its inflammatory reaction in the lungs. The wet / dry ratio and the total protein (TP) in bronchoalveolar lavage fluid were measured to understand the lung injury. Immunohistochemical studies of ICAM-1, MMP-2 and MMP-9 were performed respectively. Results 1. The ICAM-1 staining intensity of alveolar epithelial cells in both groups increased more than that in uninfected cases after Pseudomonas aeruginosa lung infection. The levels of ICAM-1 in the alveolar epithelial cells of the two groups were significantly increased at 6h, 9h and 24h after inoculation (P <0.05); 2 . After pulmonary infection of Pseudomonas aeruginosa, the MMP-2 staining of lung tissue increased in both groups and peaked at 6h or 9h after inoculation, and continued to 24h. There was no significant difference in the integral of MMP-2 between the two groups before and after infection. The change of MMP-9 before and after infection is similar to that of MMP-2; 3. ICAM-1 score of lung tissue in both groups was correlated with semi-quantitative neutrophil count (r_s = 0.641, P = 0.001 in the control group, r_s = 0.668, P <0 .001); 4. The positive staining of MMP-2 and MMP-9 in the lung tissue of both groups was positively correlated with the semi-quantitative count of PMN (P <0.01) .5. The expression of MMP-9 in the bronchial epithelial cells of immunocompromised rats was positively correlated with TP (r_s = 0.484, P <0.05). The expression of MMP-9 in alveolar epithelial cells, small vascular endothelial cells, 2 staining intensity correlated with TP (r_s = 0.457,0.492,0.429, P <0.05). Conclusion Immunohistochemical staining of ICAM-1, MMP-2 and MMP-9 in the lung tissue of immunocompromised rats after Pseudomonas aeruginosa infection is enhanced, reaching the peak at 6 ~ 9 h after infection and impaired immunity Significant neutrophil infiltration in lung tissue of rats and severe lung injury.
