目的 探讨不孕症患者输卵管在生殖生理功能中的主动运动。方法 将99mTc标记的卵子大小的人血浆白蛋白颗粒显像剂无压注入 80例临床诊断为输卵管性不孕症患者的宫腔 ,观察输卵管主动运动功能 ,以鉴别输卵管主动运动功能梗阻或机械性梗阻。结果 80例 1 6 0根输卵管主动运动功能正常者 2 7根 (1 6 .9% ) ,明显不畅者 83根 (5 1 .9% ) ,梗阻者 5 0根 (31 .3% )。对具有随访条件的 36例 72根输卵管进行了检查后的情况调查 ,有1 7例 34根 (4 7.2 % )输卵管已妊娠 ,其中运动功能显像正常者 2 1根 (6 1 .8% ) ,异常者 1 3根 (38.2 % )。对 34根输卵管均在该检查后同时进行加压通液实验 ,以除外机械性梗阻 ,其中证实机械性通畅正常者 30根 (88.2 % ) ,异常者 4根 (1 1 .8% )。结论 核素输卵管功能显像对不孕症患者输卵管的运动功能提供了一种安全、可靠的诊断方法。
Objective To explore the active movement of fallopian tubes in reproductive physiology in infertile patients. Methods 99mTc labeled egg-sized human serum albumin particle imaging agent was injected into the uterine cavity of 80 patients with tubal infertility clinically without pressure to observe the tubal active motor function to identify tubal active motor obstruction or mechanical obstruction. Results There were 27 cases (16.9%) of the 80 cases with normal fallopian tubes actively impaired, 83 (51.9%) obviously obstructive, and 50 (31.3%) obstructed. Thirty-six cases of tubal fallopian tubes with follow-up conditions were investigated. Thirty-four fallopian tubes were simultaneously subjected to pressurized fluid passage test after the test to exclude mechanical obstruction. Among them, 30 (88.2%) had normal mechanical patency and 4 (11.8%) had abnormalities. Conclusions Nuclide functional imaging provides a safe and reliable diagnostic method for tubal motor function in infertile patients.
