目的 :在Kaneda器械的基础上研制一种新型胸腰椎前路复位固定装置ARF(anteriorreductionfixation) ,以增加器械自身复位和压缩能力并方便操作。方法 :通过人尸体脊柱离体生物力学试验 ,了解ARF的稳定能力和载荷屈服强度。在此基础上ARF临床应用 6例。结果 :ARF在前屈和左侧弯方向上稳定性大于Kaneda(P <0 0 1) ,在前屈方向上低于AF(P <0 0 1) ,其余方向与两者无显著性差别 (P >0 0 5 ) ;ARF在所有方向上大于完整对照组 (P<0 0 1)。ARF平均屈服强度 (M ) 6 5 7 3公斤。临床应用早期结果 :5例椎体塌陷者基本恢复了椎体正常高度和脊柱生理弯曲 ,无神经功能恶化。结论 :ARF稳定性好 ,复位和加压能力强 ,操作方便 ,临床实用可行。
PURPOSE: To develop a novel anterior reduction fixation (ARF) on the basis of the Kaneda instrument to increase the device’s ability to reset and compress itself and facilitate its operation. Method :After in vitro biomechanical tests of human cadaveric spine, understand the stability ability and load yield strength of ARF. On this basis, 6 cases of clinical application of ARF. RESULTS: The stability of ARF in the flexion and flexion directions was greater than that of Kaneda (P <0 01) and AF in the flexion direction (P <0 01). There was no significant difference between the other directions (P <0.01). P > 0 0 5 ); ARF was greater in all directions than the intact control group (P < 0 01). ARF average yield strength (M) 6 5 7 3 kg. Clinical application of the early results: 5 cases of vertebral collapse who basically restored the normal height of the vertebral body and physiological curvature of the spine, no deterioration of nerve function. Conclusion: ARF has good stability, strong reset and pressurizing ability, convenient operation, and practical and feasible clinical application.
