目的探讨多层螺旋CT气管、支气管树三维成像在中央型肺癌诊断中的临床应用价值。方法75例中央型肺癌患者行CT容积再现(VR)及多平面重建(MPR),并与手术、纤维支气管镜(FOB)所见及病理结果相对照。结果VR能观察中央型肺癌癌肿累及气管、支气管腔的部位、形态、程度和狭窄的范围等形态改变,但不能充分显示病变本身;MPR能观察肿瘤的实质部分,肿瘤与气管、支气管的关系。结论MPR重建可显示病变本身以及病变与气管、支气管的关系;VR气管、支气管树图像直观显示气管、支气管与病变之间的关系。
Objective To investigate the clinical value of multi-slice CT tracheal and bronchial tree three-dimensional imaging in the diagnosis of central lung cancer. Methods 75 patients with central lung cancer underwent CT volume reconstruction (MPR) and multiplanar reconstruction (MPR). The results were compared with the findings of FOB and pathological findings. Results VR was able to observe the morphological changes of the central lung cancer including trachea and bronchial cavity, the shape, degree and the scope of the stenosis, but not enough to show the lesion itself. The MPR can observe the substantial part of the tumor, the relationship between the tumor and the trachea and bronchus . Conclusion Reconstruction of MPR can show the relationship between lesion and trachea and bronchus. The relationship between trachea, bronchus and lesion can be visualized by VR trachea and bronchial tree.
