髓内针固定治疗儿童股骨干骨折

目的观察髓内针固定对儿童股骨干骨折的疗效及对股骨生长的影响。方法对36例儿童股骨干骨折采用手术切开复位,小儿髓内针固定,随访0.5-3年。结果全部病例未出现手术并发症,术后6~8周达到骨性愈合,患肢与健侧等长28例,7例较健侧长0.6~1.2cm,1例缩短1.5cm,X线摄片检查未见大粗隆骨骺损伤闭合征象。结论小儿骨折髓内针固定可达到良好复位和稳固固定,只要掌握好手术适应证,小儿髓内针固定可作为治疗儿童股骨干骨折的一种有效方法。 Objective To observe the effect of intramedullary nailing on femoral shaft fractures in children and its effect on femur growth. Methods Thirty-six children with femoral shaft fractures were treated with open reduction and pedicle screw fixation. The patients were followed up for 0.5-3 years. Results No complications were found in all the cases. Bony union was achieved 6 to 8 weeks after operation. Twenty-eight cases were equal length in limb and contralateral side, 7 cases were 0.6-1.2 cm longer than healthy side, and 1.5 cm shorter in 1 case X-ray examination showed no signs of greater trochanter epiphyseal injury. Conclusion The fixation of pedicle screw fixation in pediatric children can achieve a good reduction and stable fixation. As long as the indications of operation are well, the pedicle screw fixation in children can be used as an effective method for the treatment of femoral shaft fractures in children.
