类鼻疽是一种由类鼻疽伯克霍尔德菌导致的热带医学疾病,其临床表现多样,也有“似百样病”之称,其病原已被美国CDC列为生物恐怖剂。类鼻疽在中国的海南、广西以及福建等地都有过报道,但由于其误诊率高,也未被列为法定报告的传染病,对类鼻疽的研究并未得以重视。类鼻疽菌对多种抗生素具有天然的耐药性,同时其基因组变异迅速,能产生新的耐药特性,造成的感染极易复发,治疗困难。目前临床上对类鼻疽的耐药检测还未建立统一的检测指标和判断标准,国内也还没有规范的类鼻疽抗生素用药指南。我们总结了中国2006-2014年公开发表文献中的类鼻疽临床用药及耐药结果,结合本室的类鼻疽临床资源库数据,对中国类鼻疽菌的耐药谱进行统计分析,总结用药规律,以期寻找合适的用药方案;同时,还分析了类鼻疽菌的可能耐药机制,探讨了类鼻疽耐药的深层次原因,以期为后续类鼻疽的治疗及用药指南的制定提供理论指导和临床借鉴参考。

At present, clinical trials on the drug resistance of the glanders have not yet established a unified detection index and judgment standard, and there is no standardized guideline for the application of antibiotics in the world. We summarized the clinical drug resistance and drug resistance of the Chinese gynostemma from 2006 to 2014 in our country. Based on the data of the clinical resources database of this disease, we analyzed the drug resistance spectrum of the Chinese gut bacteria and summarized the rules of drug use in order to find a suitable The possible drug resistance mechanisms of the pathogen were also analyzed and the deep reason of the drug resistance of the glanders was explored to provide theoretical guidance and clinical reference for the future treatment of glanders and the preparation of the drug guideline.
