目的了解浙江省严重精神障碍患者治疗管理人力资源概况,为区域精神卫生服务人力资源规划和政策制定提供依据。方法从人员构成、工时利用率、流失率和培训情况等方面分析杭州、台州、金华和温州4个市患者治疗管理人力资源情况。结果 4城市从事严重精神障碍患者治疗管理工作的人员分别为6 747、8 090、1 944和2 424人,共计19 305人,其中精神科医生和护士占8.39%,非精神科医生和护士占29.40%。医护人员大专及以下学历占74.77%,初级职称占54.89%,副高级职称及以上仅占0.90%。杭州市精神科医生和护士工时利用率分别为41.12%和31.81%,金华市为88.60%和87.74%,台州市为72.80%和63.45%,温州市为110.24%和121.08%。杭州市人员流失率均在5%以下,金华市公卫医生流失率达45.48%,台州市非精神科护士流失率为69.08%,温州市精神科医生流失率为22.41%,公卫医生流失率为42.59%。2015年精神科医生护士参与培训人数仅占2.55%。结论浙江省4城市严重精神障碍患者治疗管理人力资源配置欠合理,专业人员缺乏,队伍稳定性较差,培训有待加强。
Objective To understand the general situation of treatment management human resources in patients with severe mental disorders in Zhejiang Province and to provide the basis for the planning and policy formulation of human resources for regional mental health services. Methods Analyzed the human resources of treatment management in 4 cities of Hangzhou, Taizhou, Jinhua and Wenzhou from the aspects of staff composition, working hours utilization rate, wastage rate and training situation. Results There were 6 747,8 090,1 944 and 2 424 persons engaged in the treatment and management of patients with severe mental disorders in 4 cities in total, accounting for a total of 19,305 persons, of whom 8.39% were psychiatrists and nurses, while non-psychiatrists and nurses accounted for 29.40%. Medical staff and college education accounted for 74.77%, primary titles accounted for 54.89%, associate senior titles and above accounted for only 0.90%. The hourly utilization rates of psychiatrists and nurses in Hangzhou City were 41.12% and 31.81% respectively, that of Jinhua City was 88.60% and 87.74%, that of Taizhou City was 72.80% and 63.45% respectively, and that of Wenzhou City was 110.24% and 121.08%. The loss rate of staff in Hangzhou was below 5%, the loss rate of public health doctors in Jinhua reached 45.48%, the loss rate of non-psychiatric nurses in Taizhou was 69.08%, the loss rate of psychiatric doctors in Wenzhou was 22.41% 42.59%. In 2015, the number of psychiatrist nurses participating in training only accounted for 2.55%. Conclusion The treatment and management of human resources in patients with severe mental disorders in 4 cities of Zhejiang Province are not well-equipped, lack of professionals and poor team stability, and the training needs to be strengthened.
