目的探讨鼻塞式持续气道正压通气(NCPAP)联合肺表面活性物质固尔苏(Curosurf)对早产儿肺透明膜病的作用。方法NCPAP组22例患肺透明膜病的早产儿经气管内滴入Curosurf[100mg/(kg.次)],拔管予以NCPAP呼吸支持治疗;对照组22例常规机械通气病例,观察两组患儿的临床症状、体征、血气及并发症、住院费用等。结论两组患儿症状、体征均明显好转;治疗后6、12、24h,两组患儿的血气均较治疗前显著改善,但两组间无明显差异;nCPAP组肺部感染及慢性肺疾病的发生率明显低于机械通气组;NCPAP组的氧疗及住院时间明显少于机械通气组,住院费用低。结论NCPAP、Curo-surf联合应用能有效地治疗早产儿肺透明膜病。与常规机械通气相比,具有肺部感染、慢性肺疾病发生率低,住院时间短,住院费用少等优点。
Objective To investigate the effect of nasal continuous positive airway pressure (NCPAP) and pulmonary surfactant Curosurf on hyaline membrane disease in premature infants. Methods Twenty-two premature infants with hyaline membrane disease were treated with Curosurf [100 mg / (kg)] by endotracheal intubation and were given NCPAP for extubation. In the control group, 22 patients with conventional mechanical ventilation were observed. Children’s clinical symptoms, signs, blood gas and complications, hospital fees and so on. Conclusions The symptoms and signs of children in both groups improved obviously at 6, 12 and 24 hours after treatment. The blood gas of both groups improved significantly compared with that before treatment, but there was no significant difference between the two groups. The pulmonary infection and chronic lung disease In the NCPAP group was significantly lower than that in the mechanical ventilation group. The oxygen therapy and hospital stay in the NCPAP group were significantly less than those in the mechanical ventilation group and the hospitalization costs were low. Conclusions The combination of NCPAP and Curo-surf can effectively treat hyaline membrane disease in premature infants. Compared with conventional mechanical ventilation, with lung infection, low incidence of chronic lung disease, shorter hospital stay, hospitalization costs less advantages.
