目的探讨磁共振弥散成像(MRDI)对短暂性脑缺血疾病(TCID)的诊断价值。方法确诊为缺血性脑卒中患者100例,入院后给予抗凝、抗血小板、扩张血管和溶栓等治疗。随机均分为两组,分别采用MRDI脑部扫描(A组)和常规MRI(B组)。依据脑部扫描结果进行对应治疗。比较两组患者在TCID发作治疗前、治疗后以及缓解期时的阳性检出率。结果 A组患者TCID发作的阳性检出率为98.1%,高于B组的76.9%(P<0.05);A组患者TCID缓解期阳性检出率为91.9%,亦高于B组的70.3%(P<0.05)。治疗1年后,A组脑卒中的发生率4.8%,低于B组的21.6%(P<0.05);A组病灶阳性检出率75.2%,高于B组的39.4%(P<0.05)。结论 MRDI扫描法对于TCID发作的诊断具有高度的敏感性,可以指导医生尽早发现患者在脑部有关血流方面的异常情况,适时采取积极的治疗,减缓疾病发展,改善预后。
Objective To investigate the diagnostic value of magnetic resonance diffusion imaging (MRDI) in patients with transient ischemic attack (TCID). Methods One hundred patients diagnosed as ischemic stroke were given anticoagulant therapy, antiplatelet therapy, dilation of blood vessels and thrombolysis after admission. Randomly divided into two groups, MRDI brain scan (group A) and conventional MRI (group B). According to the results of brain scan corresponding treatment. The positive rates of TCID before and after treatment and remission were compared between the two groups. Results The positive rate of TCID in group A was 98.1%, which was higher than 76.9% in group B (P <0.05). The positive rate of TCID in group A was 91.9%, which was also higher than that in group B (70.3% (P <0.05). After 1 year of treatment, the incidence of stroke in group A was 4.8%, which was lower than 21.6% in group B (P <0.05). The positive rate of group A was 75.2%, which was higher than that in group B (39.4%, P <0.05) . Conclusion The MRDI scanning method is highly sensitive to the diagnosis of TCID. It can guide doctors to detect abnormalities of blood flow in the brain as soon as possible and take active treatment in time to slow the development of disease and improve the prognosis.
