目的探讨阴道封闭手术对盆腔器官脱垂(Pelvic Organ Prolapse,POP)患者排尿困扰的疗效。方法 2005年10月-2010年2月间采用盆底功能障碍症状问卷排尿困扰量表(Urinary Distress Inventory,UDI-6)和盆腔器官脱垂困扰量表(Pelvic Organ Prolapse Distress Inventory,POPDI-6)以及生活质量问卷排尿影响问卷(Urinary Impact Questionnaire,UIQ-7)和盆腔器官脱垂影响问卷(Pelvic Organ Prolapse Impact Questionnaire,POPIQ-7),对60例因POP-QⅢ-Ⅳ期重度POP实施阴道封闭手术患者,分别于术前、术后2个月、6个月及1年进行调查。结果术后2个月、6个月及1年时随访率分别为93%(56/60)、88%(53/60)和87%(52/60)。术后2月时,全部患者排尿梗阻症状消失,尿频、尿急及压力性尿失禁明显缓解并维持至术后1年。手术前后的排尿困扰症状及生活质量评分显著下降,差异有统计学意义(P=0.000 0)。结论阴道封闭手术可有效缓解POP患者排尿困扰,显著改善患者生活质量。
Objective To investigate the curative effect of vaginal closure surgery on urinary disturbance in pelvic organ prolapse (POP) patients. Methods Urinary Distress Inventory (UDI-6) and Pelvic Organ Prolapse Distress Inventory (POPDI-6) were collected from October 2005 to February 2010. Sixty patients with vaginal closure due to POP-QIII-IV severe POP were enrolled in this study. Urinary Impact Questionnaire (UIQ-7) and Pelvic Organ Prolapse Impact Questionnaire (POPIQ-7) Surgical patients were respectively preoperatively, 2 months, 6 months and 1 year after surgery. Results The follow-up rates at 2 months, 6 months and 1 year after operation were 93% (56/60), 88% (53/60) and 87% (52/60) respectively. At 2 months after operation, the symptoms of micturition obstruction disappeared in all patients. Frequent urination, urgency and stress urinary incontinence were relieved and maintained until 1 year after operation. The symptoms of urination and the quality of life before and after surgery were significantly decreased, with significant difference (P = 0.000 0). Conclusion Vaginal closure surgery can effectively relieve urination problems in patients with POP and significantly improve the quality of life of patients.
