目的应用海信CAS计算机辅助手术系统获取儿童各年龄段肝脏体积并进行大数据分析,建立中国儿童肝脏体积范围标准,为医疗和科研提供参考。方法应用海信CAS计算机辅助手术系统对696例国内儿童肝脏的CT影像资料进行三维重建,其中对增强CT进行肝脏及肝脏脉管系统、胆囊三维重建共457例(男201例,女256例),对平扫CT进行肝脏三维重建共239例(男129例,女110例)。按照年龄划分成19组,统计海信CAS计算机辅助手术系统测量的各年龄段研究对象肝脏体积及性别,最后进行统计学分析。结果同一年龄段(13~18岁年龄段除外)不同性别肝脏体积不存在差异(P>0.05)。各年龄段研究对象男女性别差异及年龄与体积线性相关关系如下:0~3岁:男女性别间无差异(P>0.05),年龄x(月)与肝脏体积y(cm~3)的正相关关系极其明显,回归方程y=9.344x+198.68(P<0.05,r=0.861 1)。3~6岁:男女性别间无差异(P>0.05),回归方程y=5.228 3x+309(P<0.05,r=0.614 2)。6~12周岁:男女性别间无差异(P>0.05),回归方程y=71.252x+272.63(P<0.05,r=0.801 0)。12~18周岁:男女性别间存在差异(P<0.05),男性肝脏体积大于女性肝脏体积,回归方程y=40.571x+657.82(P<0.05,r=0.577 4)。结论小儿肝脏体积随年龄变化而变化,这一研究初步建立了中国小儿肝脏体积的范围标准,在小儿肝脏形态学、肝脏再生、肝脏功能、肝胆外科以及小儿肝脏移植等领域具有一定的参考价值。
Objective To apply Hisense CAS computer-assisted surgery system to obtain the liver volume of children of all ages and carry out big data analysis to establish the Chinese children’s liver volume range standard, which provides a reference for medical and scientific research. Methods A total of 457 cases (201 males and 256 females) of liver and liver vasculature and three-dimensional reconstruction of gallbladder with enhanced CT were retrospectively analyzed with Hisense CAS computer-assisted surgery system on 696 children with CT images of children’s liver. A total of 239 cases (129 males and 110 females) were reconstructed by plain CT. According to the age, the patients were divided into 19 groups, and the liver volume and gender of all age groups measured by His Computer CAS were calculated. Finally, statistical analysis was made. Results In the same age group (except 13 to 18 years of age) there was no difference in liver volume between different sexes (P> 0.05). There was no significant difference between men and women (P> 0.05), and there was a positive correlation between age x (month) and liver volume y (cm ~ 3) The relationship is very obvious, the regression equation y = 9.344x + 198.68 (P <0.05, r = 0.861 1). 3 to 6 years old: no difference between male and female (P> 0.05), regression equation y = 5.228 3x + 309 (P <0.05, r = 0.614 2). 6 ~ 12 years old: no difference between male and female (P> 0.05), the regression equation y = 71.252x + 272.63 (P <0.05, r = 0.801 0). The age of 12 ~ 18 years old was different between male and female (P <0.05). The male liver volume was larger than the female liver volume. The regression equation y = 40.571x + 657.82 (P <0.05, r = 0.577 4). Conclusions The volume of liver in children changes with age. This study initially established the range standard of Chinese children ’s liver volume, which is of certain reference value in the fields of pediatric liver morphology, liver regeneration, liver function, hepatobiliary surgery and pediatric liver transplantation.
