目的分析携带地中海贫血基因对手工分离血小板的影响。方法对10例地中海贫血献血者与10例健康对照组制备浓缩血小板,比较制备血小板数目、血小板得率、红细胞残留量,并与红细胞参数行相关分析。结果地贫组与正常对照组血小板得率无显著性差异(P=0.164);携带地贫基因时,制备的血小板中的残存红细胞计数高于对照组(P=0.000)。残留红细胞计数与平均红细胞体积(mean corpuscular volume,MCV)、平均血红蛋白量(mean corpus-cular hemoglobin,MCH)呈负相关(分别为r=-0.651,P=0.002;r=-0.633,P=0.003),与红细胞分布宽度(redblood cell distribution width,RDW)呈正相关(r=-0.748,P=0.000)。结论携带地贫基因时不影响分离血小板效率,但增加红细胞的污染率,MCV越低者,红细胞污染越大。在制备战备浓缩血小板时,应尽量避免采用低MCV者,在地贫高发区制备血小板时尤应注意。
Objective To analyze the effect of carrying thalassemia gene on platelets isolated by hand. Methods Platelet was prepared from 10 cases of thalassemia in blood donors and 10 healthy controls. The number of platelets, the yield of platelets and the residual amount of erythrocytes were compared, and the correlations were analyzed with erythrocyte parameters. Results There was no significant difference in platelet yield between thalassemia group and normal control group (P = 0.164). In the case of thalassemia gene, the residual erythrocyte count in prepared platelets was higher than that in control group (P = 0.000). Residual erythrocyte counts were negatively correlated with mean corpuscular volume (MCV) and mean corpus-hemoglobin (MCH) (r = -0.651, P = 0.002; r = -0.633, P = 0.003 ), And red blood cell distribution width (RDW) were positively correlated (r = -0.748, P = 0.000). Conclusion When carrying thalassemia gene, it does not affect the efficiency of platelet separation, but increases the contamination rate of erythrocytes. The lower the MCV, the greater the erythrocyte contamination. In the preparation of warplanes concentrated platelets, should be avoided as low MCV, especially in the preparation of platelets in high thalassemia areas should pay attention.
