目的:通过检测脑卒中后抑郁(poststroke depression,PSD)患者血浆同型半胱氨酸(Homocysteine,Hcy)、超敏C反应蛋白(High sensitive creactive protein,hs-CRP)的水平,探讨护理干预后患者血浆Hcy、hs-CRP水平的变化与PSD之间的关系。方法:选择2010年6月至2014年6月住院的82例PSD患者随机分为实验组和对照组,对照组予以常规治疗和护理,实验组是在常规治疗的基础上综合性护理干预;采用汉密尔顿抑郁量表(HAMD)进行抑郁程度的评定。结果:对PSD予以心理护理干预后2周的血浆Hcy浓度为(18.67±0.62)μmo L/L,对照组为(19.02±0.87)μmo L/L,两组比较有统计学意义(P<0.05);hs-CRP浓度为(8.66±0.83)mg/L,对照组为(9.02±0.63)mg/L,两组比较有统计学意义(P<0.05)。4周的血浆Hcy浓度为(16.66±0.43)μmo L/L,对照组为(18.05±0.41)μmo L/L,两组比较差异显著,有统计学意义(P<0.01);hs-CRP浓度为(6.98±0.41)mg/L,对照组为(7.65±0.75)mg/L,两组比较差异显著,有统计学意义(P<0.01)。结论:心理护理干预可降低Hcy、hs-CRP水平,从而能够降低PSD的发生,可改善PSD的病情。
Objective: To investigate the levels of plasma homocysteine (Hcy) and high sensitive creactive protein (hs-CRP) in patients with post-stroke depression (PSD) Plasma Hcy, hs-CRP level changes and the relationship between the PSD. Methods: Eighty-two PSD patients admitted to our hospital from June 2010 to June 2014 were randomly divided into experimental group and control group. The control group was given routine treatment and nursing. The experimental group was treated with general nursing intervention. Depression scale was assessed by Hamilton Depression Rating Scale (HAMD). Results: Plasma homocysteine (Hcy) concentration was (18.67 ± 0.62) μmo L / L in two weeks after intervention of PSD, and (19.02 ± 0.87) μmo L / L in control group, with statistical significance (P <0.05 ). The concentration of hs-CRP was (8.66 ± 0.83) mg / L in the control group and (9.02 ± 0.63) mg / L in the control group, with statistical significance (P <0.05). The concentration of plasma Hcy was (16.66 ± 0.43) μmo L / L in 4 weeks and (18.05 ± 0.41) μmo L / L in control group, with significant difference between the two groups (P <0.01) (6.98 ± 0.41) mg / L in the control group and (7.65 ± 0.75) mg / L in the control group. There was significant difference between the two groups (P <0.01). Conclusion: Psychological nursing intervention can reduce the levels of Hcy and hs-CRP, which can reduce the occurrence of PSD and improve the condition of PSD.
