目的:探讨畸胎癌衍生生长因子1(Cripto-1)在胰腺导管腺癌组织中的表达与临床意义。方法:采用免疫组化法检测62例胰腺导管腺癌组织中Cripto-1的表达,结合临床病理特征进行分析。结果:Cripto-1在胰腺导管腺癌组织中的阳性率为67.7%。高、中、低分化导管腺癌病人中的Cripto-1阳性率分别为27.3%、67.6%、94.1%,差异有统计学意义(P<0.05)。无血管侵犯和有血管侵犯病人中的Cripto-1阳性率分别为58.7%和93.8%,差异有统计学意义(P<0.05)。无淋巴结转移和有淋巴结转移的病人中的Cripto-1阳性率分别为53.8%和91.3%,差异有统计学意义(P<0.05)。Cripto-1高表达组病人的术后生存期短于Cripto-1低表达组的病人,差异有统计学意义(P<0.05)。COX多因素分析表明,Cripto-1表达阳性、临床JPS高分期和淋巴结转移是反映术后生存率的独立因素。结论:Cripto-1表达水平可能与胰腺导管腺癌的发生、发展、侵袭及转移相关,其确切的分子生物学机制有待进一步研究。
Objective: To investigate the expression and clinical significance of Cripto-1 in pancreatic ductal adenocarcinoma. Methods: Immunohistochemistry was used to detect the expression of Cripto-1 in 62 cases of pancreatic ductal adenocarcinoma, and the clinicopathological characteristics were analyzed. Results: The positive rate of Cripto-1 in pancreatic ductal adenocarcinoma was 67.7%. The positive rates of Cripto-1 in high, moderate and poorly differentiated ductal adenocarcinoma were 27.3%, 67.6% and 94.1%, respectively, with significant difference (P <0.05). The positive rates of Cripto-1 in non-vascular invasion and vascular invasion were 58.7% and 93.8% respectively, with statistical significance (P <0.05). The positive rates of Cripto-1 in patients without lymph node metastasis and with lymph node metastasis were 53.8% and 91.3%, respectively, with statistical significance (P <0.05). The patients with high Cripto-1 expression group had shorter survival time than those with low Cripto-1 expression group (P <0.05). COX multivariate analysis showed that the positive expression of Cripto-1, high clinical stage of JPS and lymph node metastasis is an independent factor reflecting the postoperative survival rate. Conclusion: The expression of Cripto-1 may be related to the occurrence, development, invasion and metastasis of pancreatic ductal adenocarcinoma. The exact molecular biological mechanism remains to be further studied.
