目的 研究自体掌长肌游离移植治疗恒河猴括约肌无力性尿失禁的机制 ,为临床提供依据。方法 将预先去神经 2周的自体掌长肌游离移植至尿失禁猴的后尿道段 ,以加强尿道括约肌的功能 ,并于移植后不同时期观察其排尿状况、尿流动力学有关指标并做移植肌的组织学检查。结果 2 0只实验猴在移植术后 1、3、5个月排尿状况及尿流动力学指标逐步得到改善 ,至术后 5个月时 ,可见再生神经纤维和运动终板 ,肌纤维排列结构正常。除 1只母猴在剧烈运动中偶见漏尿外 ,其余均已痊愈。结论 本手术方法具有成功率高 ,并发症少等特点 ,能有效治疗恒河猴括约肌无力性尿失禁 ,可供临床参考
Objective To study the mechanism of autologous palmaris muscle transplantation in treatment of incontinence of rhesus sphincter in order to provide evidence for clinical practice. Methods The autograft palmar muscle, which had been denervated for 2 weeks, was transplanted into the posterior urethra of urinary incontinence monkeys in order to enhance the function of the urethral sphincter. The urinary status and urodynamic indexes were observed at different stages after transplantation and were used as explant Histological examination. Results Urine status and urodynamic indexes of 20 experimental monkeys gradually improved at 1, 3 and 5 months after transplantation. At 5 months after operation, regenerated nerve fibers and motor end plates were observed, and the arrangement of muscle fibers was normal. All but one females occasionally leak urine during strenuous exercise. Conclusion The surgical method has the characteristics of high success rate and few complications and can effectively treat rhesus sphincter invalidity incontinence for clinical reference
