目的研究血液流变学的检测在糖尿病肾病诊断中的应用价值。方法对98例糖尿病肾病(DN组)患者和88例单纯糖尿病未合并肾脏病变(NDN组)患者的血液流变学进行检测并分析结果,另选择100例在同一时间参加体检的健康者作为对照组(NC组)进行比较。结果与NC组比较,DN组和NDN组反映血液黏度的指标都显著升高(P<0.01),且DN组要显著高于NDN组(P<0.01)。与NC组比较,DN组反映红细胞聚集的指标显著升高(P<0.01),反映红细胞变形能力的指标显著降低(P<0.01);NDN组反映红细胞聚集的指标明显升高(P<0.05),反映红细胞变形能力的指标明显降低(P<0.05);与NDN组比较,DN组反映红细胞聚集的指标明显升高(P<0.05),反映红细胞变形能力的指标明显降低(P<0.05)。结论糖尿病患者体内血液黏度升高导致的微血管病变是造成糖尿病肾病的重要原因,准确监测患者血液流变学的变化能够对临床中防治糖尿病肾病具有重要意义。
Objective To study the value of hemorheology in the diagnosis of diabetic nephropathy. Methods The hemorheology of 98 patients with diabetic nephropathy (DN group) and 88 patients with simple non-diabetic nephropathy (NDN group) were detected and analyzed. Another 100 healthy subjects who took physical examination at the same time were selected as control Group (NC group) for comparison. Results Compared with NC group, the indexes of blood viscosity in DN group and NDN group were significantly increased (P <0.01), and were significantly higher in DN group than NDN group (P <0.01). Compared with NC group, the index of erythrocyte aggregation in DN group was significantly higher (P <0.01), the index of erythrocyte deformability was significantly lower (P <0.01), the index of erythrocyte aggregation in NDN group was significantly higher (P <0.05) (P <0.05). Compared with NDN group, the index of erythrocyte aggregation in DN group was significantly increased (P <0.05), and the index of erythrocyte deformability was significantly decreased (P <0.05). Conclusions The microvascular diseases caused by the increase of blood viscosity in diabetic patients are the important causes of diabetic nephropathy. To accurately monitor the changes of hemorrheology in patients with diabetic nephropathy is of great significance in the prevention and treatment of diabetic nephropathy.
