目的探讨慢性阻塞性肺疾病(COPD)患者MSCT定量测量肺密度及肺容积指标与肺功能指标的关系。方法选取2014年3月-2015年3月60例COPD患者为COPD组,60例健康志愿者为对照组,均行胸部MSCT检查及肺功能检查。测量并比较COPD组和对照组MSCT定量测量指标及肺功能指标的差异,观察COPD患者MSCT定量测量指标与肺功能指标的相关性。结果 COPD组深呼气末全肺容积(Vex)、深吸气末全肺容积(Vin)、肺容积比(Vex/Vin)均显著高于对照组,深呼气末平均肺密度(MLDex)、深吸气末平均肺密度(MLDin)、肺容积差(Vin-Vex)、肺密度差(MLDex-MLDin)均显著低于对照组,差异具有统计学意义(P<0.05);COPD组第1秒用力呼气量(FEV1)、用力肺活量(FVC)、第1秒用力呼气量与用力肺活量百分比(FEV1/FVC)均显著高于对照组,肺总量(TLC)、残气量(RV)、残气量与肺总量百分比(RV/TLC)均显著低于对照组,差异均有统计学意义(P<0.05);经相关性分析显示,MLDex-MLDin与肺功能指标FEV1、FVC及FEV1/FVC呈正相关(P<0.05),RV/TLC呈负相关(P<0.05)。Vex/Vin与肺功能指标RV/TLC存在正相关(P<0.05),Vin-Vex与FVC具有正相关(P<0.05)。结论 COPD患者MSCT定量测量肺密度及肺容积指标均出现明显的改变,并与临床肺功能检测具有较好的一致性。
Objective To investigate the relationship between lung density and lung volume index and lung function in MSCT patients with chronic obstructive pulmonary disease (COPD). Methods From March 2014 to March 2015, 60 COPD patients were selected as COPD patients and 60 healthy volunteers as control group. All patients underwent thoracic MSCT and pulmonary function tests. The differences of MSCT quantitative indexes and lung function indexes between COPD group and control group were measured and compared. The correlation between MSCT quantitative indexes and pulmonary function indexes was observed. Results In the COPD group, the total end-expiratory lung volume (Vex), deep end-inspiratory lung volume (Vin), and lung volume ratio (Vex / Vin) were significantly higher than those in the control group , MLDin, Vin-Vex and MLDex-MLDin in the control group were significantly lower than those in the control group (P <0.05) The forced expiratory volume (FEV1), forced vital capacity (FVC), forced expiratory volume at 1 second and forced vital capacity (FEV1 / FVC) were significantly higher than those in control group, total pulmonary volume (TLC), residual volume ), Residual volume and total lung volume (RV / TLC) were significantly lower than the control group, the difference was statistically significant (P <0.05); correlation analysis showed that MLDex-MLDin and pulmonary function parameters FEV1, FVC and FEV1 / FVC was positively correlated (P <0.05), RV / TLC was negatively correlated (P <0.05). There was a positive correlation between Vex / Vin and RV / TLC (P <0.05), and there was a positive correlation between Vin-Vex and FVC (P <0.05). Conclusion Quantitative measurement of MSCT in patients with COPD lung density and lung volume indicators were significantly changed, and clinical lung function tests with good consistency.
