慢性胰腺炎(chronic pancreatitis,CP)是由于胰腺炎症导致胰腺组织不可逆的毁损,最终将导致胰腺内、外分泌功能的部分或全部丧失。CP治疗目标是减轻疼痛,改善胰腺脂肪泻及营养不良,治疗糖尿病等并发症,保持或改善病人生活质量等。缓解疼痛是最常见的临床问题,补充胰酶和适当使用镇痛药物是常用治疗方法。胰酶制剂能显著改善脂肪泻,而其使用剂量和使用方法非常重要。近年来内镜治疗的进步令人鼓舞,方法包括胰腺括约肌切开术,支架置入术和体外震波碎石术等。内镜技术的进步为CP病人提供了新的治疗希望,但长期疗效仍有待随访。此外,在生活方式上,戒烟酒对CP病人很可能有益,而低脂饮食因为可能导致脂溶性维生素的缺乏而不被推荐。
Chronic pancreatitis (CP) is an irreversible loss of pancreatic tissue due to inflammation of the pancreas that eventually leads to the loss of some or all of the exocrine and endocrine functions of the pancreas. CP treatment goals are to reduce pain, improve pancreatic steatorrhea and malnutrition, diabetes and other complications, to maintain or improve the quality of life of patients. Pain relief is the most common clinical problem, supplementation of pancreatic enzymes and the appropriate use of analgesics is a common treatment. Pancreatic enzymes can significantly improve steatorrhea, and its use of dosage and method of use is very important. In recent years, the progress of endoscopic treatment is encouraging, including pancreatic sphincterotomy, stent implantation and extracorporeal shock wave lithotripsy. Advances in endoscopic techniques have offered new therapeutic hope for CP patients, but long-term efficacy remains to be followed up. In addition, in the way of life, quit smoking alcohol is likely to benefit patients with CP, and low-fat diet because of the lack of fat-soluble vitamins may not be recommended.
