目的:就无创心血管超声在肺动脉栓塞诊断中的临床应用价值进行探讨。方法:选取我院2013年5月至2014年2月所收治的肺动脉栓塞160例患者,仪器采用频率为1~5MHz、探头为S5-1的Philips IE33彩色多普勒超声诊断仪,周围血管探头频率为3~11MHz。结果:80例右肺动脉内径异常(16-29mm),76例主肺动脉内径异常(30-39mm),108例右心房上下径异常(49-70mm),100例右心室左右径异常(39-58mm),96例右心房左右径异常(40-66mm)。反流程度为中轻度肺动脉栓塞患者的三尖瓣反流速度及肺动脉收缩压都要明显低于反流程度为重度的肺动脉栓塞患者,具有较为明显的差异,存在着统计学意义(P<0.05)。结论:无创心血管超声检查价格低廉、无创伤、实用简便,对于早期诊断、早期治疗肺动脉栓塞具有较大的价值,值得推广。
Objective: To investigate the clinical value of noninvasive cardiovascular ultrasound in the diagnosis of pulmonary embolism. Methods: 160 patients with pulmonary embolism admitted from May 2013 to February 2014 in our hospital were selected. Philips IE3300 color Doppler ultrasound scanner with frequency of 1 ~ 5MHz and probe S5-1 was used. Peripheral vascular probe The frequency is 3 ~ 11MHz. Results: There were 80 patients with abnormal diameter of right pulmonary artery (16-29mm), 76 patients with abnormal diameter of main pulmonary artery (30-39mm), 108 patients with right atrium abnormality (49-70mm), 100 patients with right and left ventricular dysfunction (39-58mm ), 96 cases of right atrial diameter abnormalities (40-66mm). Reflux degree of patients with mild to moderate pulmonary embolism in the tricuspid regurgitation velocity and pulmonary artery systolic pressure were significantly lower than the degree of severe reflux of pulmonary embolism patients, with a more significant difference, there is a statistically significant (P < 0.05). Conclusion: Noninvasive cardiovascular ultrasound is inexpensive, noninvasive and practical. It is of great value to early diagnosis and early treatment of pulmonary embolism. It is worth to be popularized.
