目的探讨布地奈德雾化吸入联合阿奇霉素治疗小儿支原体肺炎(MPP)的临床疗效。方法选取行唐县安香卫生院2015年1月—2016年6月收治的MPP患儿97例,随机分为对照组49例和研究组48例。患者入院后均予以对症支持治疗,对照组患儿给予阿奇霉素治疗,研究组患儿在对照组基础上给予雾化吸入布地奈德混悬液治疗。治疗后14 d,比较两组患儿的临床疗效,体温恢复正常时间及肺部湿啰音消失时间,治疗前及治疗7 d实验室检查指标[C反应蛋白(CRP)、重组人粒细胞刺激因子(G-CSF)、可溶性白介素2受体(s IL-2R)]水平,并观察比较两组患儿治疗期间不良反应发生情况。结果研究组患儿临床疗效优于对照组(P<0.05)。研究组患儿体温恢复正常时间、肺部湿啰音消失时间短于对照组(P<0.05)。治疗前,两组患儿CRP、G-CSF、s IL-2R水平比较,差异无统计学意义(P>0.05);治疗7 d,研究组患儿CRP、G-CSF、s IL-2R水平低于对照组(P<0.05)。两组患儿不良反应发生率,差异无统计学意义(P>0.05)。结论布地奈德雾化吸入联合阿奇霉素治疗小儿MPP的临床疗效确切,可明显缩短症状体征改善时间,促进疾病转归,且安全性好。
Objective To investigate the clinical efficacy of budesonide inhalation combined with azithromycin in the treatment of children with mycoplasma pneumonia (MPP). Methods A total of 97 MPP children admitted to AnXiang Hospital of Tangxian County from January 2015 to June 2016 were randomly divided into control group (n = 49) and study group (n = 48). Patients were admitted to symptomatic and supportive treatment, control group of children given azithromycin treatment, the study group were given the control group on the basis of inhalation of budesonide suspension treatment. On the 14th day after treatment, the clinical curative effect, the normal time of body temperature recovery and the disappearance time of the pulmonary wet rales were compared between the two groups. Before the treatment and on the 7th day after treatment, the laboratory test indexes [C-reactive protein (CRP), recombinant human granulocyte stimulation (G-CSF) and soluble interleukin-2 receptor (sIL-2R)] levels were observed and compared between two groups of children treated during adverse reactions. Results The clinical efficacy of study group was better than that of control group (P <0.05). Study group children with normal body temperature recovery, lung wet rales disappeared shorter than the control group (P <0.05). The levels of CRP, G-CSF and sIL-2R in the two groups were not significantly different before treatment (P> 0.05) Lower than the control group (P <0.05). There was no significant difference in adverse reactions between the two groups (P> 0.05). Conclusion Budesonide inhalation combined with azithromycin in the treatment of pediatric MPP has an exact clinical curative effect, which can obviously shorten the time of improvement of symptoms and signs, promote the prognosis of the disease, and have good safety.
