目的研究2型糖尿病患者中Hb A1c与细胞抗氧化水平、脂质过氧化状态之间的关系。方法将60例糖尿病患者和20名健康正常人分为:组1:正常对照组,组2:患者Hb A1c<7.0%,组3:患者7.0%9.0%。测定血糖、Hb A1c、SOD、GSH-Px、LPO等指标。计量资料采用方差分析,相关性分析采用Spearman相关分析,P<0.05为差异有统计学意义。结果四组研究对象的血糖[(5.15±0.45)、(6.12±0.38)、(7.99±0.57)、(10.84±1.06)mmol/L]、SOD[(1 954.2±126.1)、(1 766.8±117.6)、(1 598.3±106.2)、(1 304.1±101.8)μ/g]、GSH-Px[(30.8±4.6)、(27.2±3.2)、(24.9±4.5)、(21.0±3.1)μ/g]、LPO含量[(10.5±1.4)、(11.2±1.2)、(13.0±0.9)、(15.8±1.1)mmol/L]比较,差异均有统计学意义(F=282.08、54 906.42、497.30、515.90,均P<0.05)。两两比较发现,任2组血糖比较,均差异均有统计学意义(均P<0.05);组2,组3,组4与组1的SOD、GSH-Px活性及LPO含量比较,差异均有统计学意义(均P<0.05);组2、组3与组4的SOD、GSH-Px活性及LPO含量比较,差异均有统计学意义(均P<0.05)。Hb A1c与血糖、LPO含均呈正相关(r=0.944、0.662,均P<0.05);Hb A1c与SOD、GSH-Px含量均呈负相关(r=-0.752、-0.639,均P<0.05)。结论 Hb A1c可以反映细胞内外的氧化和抗氧化状态,这在临床及分子水平预示了糖尿病并发症的发生。
Objective To investigate the relationship between Hb A1c and anti-oxidative status and lipid peroxidation in type 2 diabetic patients. Methods Sixty diabetics and 20 normal controls were divided into two groups: group 1: normal control group, group 2: patients with Hb A1c <7.0%, group 3: patients with 7.0% 9.0%. Blood glucose, HbA1c, SOD, GSH-Px, LPO and other indicators were measured. Measurement data using analysis of variance, correlation analysis using Spearman correlation analysis, P <0.05 for the difference was statistically significant. Results The levels of plasma glucose ([(5.15 ± 0.45), (6.12 ± 0.38), (7.99 ± 0.57), (10.84 ± 1.06) mmol / L] and SOD [(1 954.2 ± 126.1), (1766.8 ± 117.6 ), GSH-Px [(30.8 ± 4.6), (27.2 ± 3.2), (24.9 ± 4.5), (21.0 ± 3.1) μ / g ], LPO content [(10.5 ± 1.4), (11.2 ± 1.2), (13.0 ± 0.9), (15.8 ± 1.1) mmol / L] 515.90, all P <0.05). The differences of blood glucose between the two groups were statistically significant (all P <0.05). The differences of SOD, GSH-Px activities and LPO contents between groups 2, 3, 4 and 1 (P <0.05). There were significant differences in SOD, GSH-Px activity and LPO content between groups 2, 3 and 4 (all P <0.05). Hb A1c had a positive correlation with blood glucose and LPO (r = 0.944,0.662, all P <0.05); Hb A1c had a negative correlation with SOD and GSH-Px (r = -0.752, -0.639, all P <0.05) . Conclusion Hb A1c can reflect the oxidation and anti-oxidation status inside and outside the cell, which indicates the occurrence of diabetic complications at the clinical and molecular levels.
