目的探讨第一产程异常胎心监护图形与围生儿结局的关系。方法对我院足月单胎头位分娩产妇第一产程胎心监护图形300例进行观察、记录和分析。结果第一产程异常FHR图形的发生率为41.33%。常见类型依次为重度早发减速(ED)、中重度变化减速(VD)、晚期减速(LD)、基线变异减弱以及胎心率基线异常(包括胎心率过速、胎心率过缓等)。其中重度变化减速、晚期减速、基线变异减弱与新生儿窒息的发生相关。观察组阴道手术产、剖宫产率明显高于对照组(P<0.05)。结论第一产程异常FHR图形的发生率较高,其中重度变异减速、晚期减速、基线变异减弱与新生儿窒息的发生相关,其他图形可在严密监护下继续试产。
Objective To investigate the relationship between abnormal fetal heart rate monitoring graph and perinatal outcome in the first stage of labor. Methods 300 cases of fetal heart rate monitoring in the first stage of delivery of full-term single-head mothers in our hospital were observed, recorded and analyzed. Results The incidence of abnormal FHR in the first stage of labor was 41.33%. The common types were severe early-onset deceleration (ED), moderate-severe deceleration (VD), late deceleration (LD), baseline variation weakened and baseline abnormal fetal heart rate (including fetal heart rate, . Among them, severe decelerations, late decelerations and baseline variability were associated with neonatal asphyxia. Observation group vaginal surgery, cesarean section rate was significantly higher than the control group (P <0.05). Conclusions The incidence of abnormal FHR pattern in the first stage of labor is high. Among them, severe variant decelerations, late decelerations and baseline variability are related to the occurrence of neonatal asphyxia. Other graphs can be further trial-produced under close supervision.
