目的探讨不同年龄组男性总有列腺特异性抗原(tPSA)水平检测及其临床意义。方法将588名男性体检者根据年龄分为4组:青年组(<45岁)125例,中年组(45~60岁)142例,普通老年组(60~75岁)169例和高龄老年组(≥75岁)152例;分析各年龄组的前列腺疾病情况和tPSA水平;各年龄组不同前列疾病情况的tPSA总水平及在不同水平(≤4 ng/ml、4~10 ng/ml和>10 ng/ml)的分布情况。结果①青年组和中年组的水平高于老年组,前列腺疾病的发生率和tPSA水平均低于老年组(P<0.01);高龄老年组的水平低于普通老年组(P<0.01),前列腺癌发生率(P<0.05)和tPSA(P<0.01)高于普通老年组;②各年龄组及总体依据前列腺疾病严重情况tPSA水平均上升,且两两之间差异有统计学意义;青年和中年两组的正常者和前列腺炎的tPSA水平均低于老年组,高龄老年组良性前列腺增生的tPSA水平高于中年组和普通老年组;③随着前列腺疾病严重程度的增加,各组tPSA水平≤4 ng/ml的分布降低;tPSA水平≥10 ng/ml的分布增多;随年龄增大,各组对应正常和疾病情况的tPSA水平≤4 ng/ml的分布降低,tPSA水平≥10 ng/ml的分布增多。结论 tPSA水平在不同年龄组同一类型前列腺疾病和同年龄段不同前列腺疾病的水平不同,提示tPSA水平受年龄因素的影响,tPSA水平的检查可用于前列腺疾病的初期诊断。
Objective To investigate the levels of total serum prostate-specific antigen (tPSA) in different age groups and its clinical significance.
