目的探讨血清内脂素(visfatin)、肿瘤坏死因子-α(TNF-α)的水平变化对多囊卵巢综合征(PCOS)发病的意义。方法收集PCOS患者50例(实验组)及对照组患者48例,并按体质量指数(BMI)将其分别分成肥胖(BMI≥25 kg/m2)和非肥胖(BMI<25 kg/m2)两个亚组,其中实验组肥胖者23例,非肥胖者27例,对照组肥胖者20例,非肥胖者28例。取空腹血清,采用酶联免疫吸附法(ELISA)测定血清visfatin水平、放射免疫法测定TNF-α水平,葡萄糖氧化酶法测定空腹血糖(FPG)、化学发光法测定空腹胰岛素(FIN)水平,同时测量身高、体质量,并计算BMI和胰岛素抵抗指数(HOMA-IR)。结果实验组visfatin、TNF-α水平均高于对照组(P<0.05)。实验组肥胖者及非肥胖者visfatin水平均高于对照组非肥胖者,实验组肥胖者visfatin水平高于对照组肥胖者;实验组肥胖者及非肥胖者TNF-α水平分别高于其相应对照组,且非肥胖实验组高于肥胖对照组(P<0.05)。实验组visfatin、TNF-α水平与BMI、HOMA-IR均呈正相关(P<0.05)。结论 PCOS患者血清visfatin、TNF-α水平升高均与胰岛素抵抗密切相关,visfatin、TNF-α可能参与了PCOS患者肥胖和胰岛素抵抗的发生发展。
Objective To investigate the significance of serum visfatin and tumor necrosis factor-α (TNF-α) in the pathogenesis of polycystic ovary syndrome (PCOS). Methods Fifty patients with PCOS (experimental group) and 48 patients with control group were divided into two groups according to the body mass index (BMI): obesity (BMI≥25 kg / m2) and non-obesity (BMI <25 kg / m2) A subgroup, of which 23 cases of obesity in the experimental group, non-obese in 27 cases, 20 cases of obesity in the control group, non-obese in 28 cases. Results The levels of visfatin and TNF-α in the experimental group were significantly higher than those in the control group (P <0.05). The levels of visfatin in obese and non-obese subjects in the experimental group were significantly higher than those in the non-obese subjects in the control group. The levels of visfatin in the experimental obese group were higher than those in the control obese group. The levels of TNF-α in the obese and non-obese subjects in the experimental group were significantly higher than those in the control group Group, and non-obese experimental group than obese control group (P <0.05). The levels of visfatin and TNF-α in experimental group were positively correlated with BMI and HOMA-IR (P <0.05). Conclusion Serum levels of visfatin and TNF-α in patients with PCOS are closely related to insulin resistance. Visfatin and TNF-α may be involved in the development of obesity and insulin resistance in PCOS patients.
