目的探讨时间-空间相关成像技术结合M型超声(Spatio-temporal image correlation with M-mode display,STIC-M)在妊娠期高血压疾病胎儿心室收缩功能评价中的应用价值。方法选择孕20~40周妊娠期高血压疾病孕妇与血压正常孕妇为研究对象。妊娠期高血压疾病孕妇根据高血压严重程度分为妊娠期高血压组(A组)、轻度子痫前期组(B组)和重度子痫前期组(C组);血压正常孕妇为对照组(D组)。应用STIC-M测量胎儿左、右心室缩短分数(Fractional shortening,FS),比较各组胎儿左、右心室FS值。结果 A组21例、B组25例、C组22例、D组34例纳入比较分析,C组胎儿右室FS值较其余各组降低(P<0.05),其余各组之间右室FS值两两比较,差异无显著性(P>0.05),4组胎儿左室FS差异均无显著性(P>0.05)。结论 STIC-M能够通过测量胎儿FS监测胎儿心室收缩功能,可作为评价胎儿心室收缩功能的新方法。妊娠期高血压疾病胎儿在重度子痫前期右心室收缩功能降低,在这一时期评估胎儿右心室功能、及早发现心功能异常具有十分重要的临床意义。
Objective To investigate the value of temporal-spatial correlation imaging combined with M-mode display (STIC-M) in the evaluation of fetal ventricular systolic function in patients with hypertensive disorder of pregnancy. Methods Pregnant women with gestational hypertension and pregnant women with normal blood pressure during the first 20 to 40 weeks of pregnancy were selected as the study objects. According to the severity of hypertension, pregnant women with gestational hypertension were divided into two groups: hypertensive pregnancy group (group A), mild preeclampsia group (group B) and severe preeclampsia group (group C) (Group D). Fetal left and right ventricular fractional shortening (Fractional shortening, FS) were measured by STIC-M, and the left and right ventricular FS values were compared between groups. Results Among 21 cases in group A, 25 cases in group B, 22 cases in group C and 34 cases in group D, the value of right ventricular FS in group C was lower than that in other groups (P <0.05) There was no significant difference between the two groups (P> 0.05). There was no significant difference in left ventricular FS among four groups (P> 0.05). Conclusion STIC-M can measure fetus ventricular systolic function by measuring fetal FS, which can be used as a new method to evaluate fetal ventricular systolic function. Hypertensive disorders of pregnancy fetus in severe preeclampsia decreased right ventricular systolic function, in this period to assess fetal right ventricular function, early detection of cardiac dysfunction has very important clinical significance.
