目的探讨Vibrant动态增强联合MR血管成像在乳腺良恶性病变诊断中应用价值。方法 36例患者共40个病灶,其中恶性病灶18个,良性病灶22个,行乳腺动态增强MRI检查,并重建得到3D-MIP图像。在GE ADW 4.3图像工作站运用Functool功能软件对增强病变进行血流动力学分析。结果以Ⅱ~Ⅲ型曲线,信号增强率>90%,峰值时间出现在3 min之前,周围血管征阳性作为恶性标准,敏感性分别为88.9%、88.9%、83.3%、77.8%,特异性分别为72.7%、54.5%、90.9%、86.4%,准确性分别为80%、70%、87.5%、82.5%,阳性预测值分别为72.7%、61.5%、88,2%、82.4%,阴性预测值分别为88.9%、85.7%、86.9%、82.6%,良恶性差异有统计学意义(P<0.05)。结论 Vibrant动态增强血流动力学特征联合周围血管征在乳腺良恶性病变判断方面具有重要价值。
Objective To investigate the value of Vibrant dynamic contrast-enhanced MR angiography in the diagnosis of benign and malignant breast lesions. Methods Totally 40 lesions were observed in 36 patients, including 18 malignant lesions and 22 benign lesions. Dynamic breast MRI was performed and 3D-MIP images were reconstructed. Hemodynamic analysis of enhanced lesions was performed using the Functool software at GE ADW 4.3 Imaging Workstation. The results showed that the rate of signal enhancement was> 90% and the peak time was before 3 min. The sensitivity of peripheral blood vessels was 88.9%, 88.9%, 83.3% and 77.8%, respectively The positive predictive value was 72.7%, 54.5%, 90.9% and 86.4%, respectively, with the accuracy of 80%, 70%, 87.5% and 82.5% respectively. The positive predictive values were 72.7%, 61.5%, 88.2% and 82.4% The values were 88.9%, 85.7%, 86.9%, 82.6% respectively. The difference between benign and malignant was statistically significant (P <0.05). Conclusions Vibrant dynamic enhanced hemodynamics combined with peripheral vascular findings is of great value in judging benign and malignant breast lesions.
