目的探讨超声造影与核磁共振(MRI)增强方式诊断乳腺病变的特点及异同。方法选取2010年11月至2014年2月间收治的经手术或穿刺活检病理诊断为乳腺病变的64例患者(共73个病灶),分析超声造影与MRI增强扫描方式的声像特征,比较二者的异同点。结果超声造影与MRI增强均匀性不完全一致(P=0.077),且超声造影(39例)较MRI(24例)增强扫描表现出更多的不均匀增强(P=0.032)。而超声造影和MRI增强后显示病变面积的一致性较好(P=0.010),且两者差异无统计学意义(P=0.338);但超声造影技术灌注缺损区面积大于MRI增强扫描,且两者差异有统计学意义(P=0.013);不均匀增强的病变面积大于均匀增强的病变面积(P<0.05)。超声造影与MRI增强时间-强度曲线类型无明显相关性。结论超声造影与MRI增强方式在乳腺病变的诊断中存在一定的相同点,但由于二者的原理不同,也存在一定的差异,临床诊断应根据二者的原理进行区分应用。
Objective To investigate the characteristics and similarities and differences of contrast-enhanced ultrasound and magnetic resonance imaging (MRI) in the diagnosis of breast lesions. Methods Sixty-four patients (73 lesions) diagnosed as mammary lesions by surgery or biopsy between November 2010 and February 2014 were selected. The sonographic features of contrast-enhanced ultrasound and enhanced MRI scans were analyzed. Some similarities and differences. RESULTS: Contrast-enhanced ultrasound and MRI enhancement uniformity were not completely consistent (P = 0.077). Contrast-enhanced ultrasound (39 cases) showed more inhomogeneous enhancement than MRI (24 cases) enhanced scan (P = 0.032). Contrast-enhanced contrast-enhanced ultrasound and MRI enhanced the consistency of the lesion area (P=0.010), and there was no significant difference between them (P=0.338). However, the area of the perfusion defect area by contrast-enhanced ultrasound was greater than that by the MRI enhanced scan, and The difference was statistically significant (P=0.013); the area of non-uniformly enhanced lesions was larger than that of uniformly enhanced lesions (P<0.05). Contrast-enhanced ultrasound and MRI enhanced time-intensity curve types were not significantly correlated. Conclusion Contrast-enhanced ultrasound and enhanced MRI have certain similarities in the diagnosis of breast lesions. However, due to the difference in the principle of the two, there are also certain differences. Clinical diagnosis should be based on the principle of the two.
