目的探讨影响昆明地区新生儿脐血C肽的母亲相关因素,以早期进行干预改善新生儿结局。方法回顾性分析昆明医科大学第一附属医院产科2014年3-8月期间单胎活产分娩的840例产妇,分析影响新生儿脐血C肽的相关母亲并发症及人口统计学参数等一般资料。结果 1脐血C肽与母亲年龄、孕产次、民族、籍贯等一般情况及人体学参数无相关性;2妊娠期甲状腺疾病与脐血C肽存在正相关(r=0.81,P=0.023);3胎盘重量与脐血C肽存在正相关关系(r=0.025,P<0.001);4系统管理妊娠期糖尿病患者后,妊娠期糖尿病对新生儿脐血C肽值无影响。结论妊娠期甲状腺疾病可能与新生儿胰岛素抵抗相关,应加强对妊娠期甲状腺疾病的管理;系统管理妊娠期糖尿病患者,对改善新生儿胰岛素抵抗有显著的积极作用。
Objective To explore the related factors that affect the mother’s umbilical cord blood C-peptide in Kunming area to improve the neonatal outcome by early intervention. Methods A retrospective analysis of 840 maternal births delivered from January to August 2014 in the obstetrics department of the First Affiliated Hospital of Kunming Medical University was conducted to analyze general information such as maternal complications and demographic parameters that influence neonatal cord C peptide . Results 1 There was no correlation between umbilical blood C-peptide and general status of mothers, motherhood, ethnicity and native place, and anthropometric parameters.2 There was a positive correlation between thyroid disease and C peptide in pregnancy (r = 0.81, P = 0.023) ; 3 There was a positive correlation between placental weight and C-peptide in cord blood (r = 0.025, P <0.001); 4 After administration of gestational diabetes in patients with gestational diabetes mellitus, gestational diabetes had no effect on the C peptide of neonatal cord blood. Conclusions Thyroid disease during pregnancy may be related to neonatal insulin resistance. Management of thyroid disease during pregnancy should be strengthened. Systematic management of gestational diabetes patients has a significant positive effect on improving neonatal insulin resistance.
