目的研究P波离散度(Pd)与冠心病并发阵发性心房颤动(PAF)的关系。方法观察112例冠心病并发PAF患者(观察组)的Pd和P波最大时限(P_(max)),并与84例无PAF的冠心病患者(对照组)的Pd、P_(max)进行比较。结果观察组Pd和P_(max)与对照组比较,差异有统计学意义(P<0.01),Pd或Pd+P_(max)对冠心病并发阵发性心房颤动有较高的特异度(82.1%、90.5%)和阳性预测价值(86.4%、91.5%)。结论Pd是预测冠心病并发阵发性心房颤动的一个可靠的体表心电图指标。
Objective To study the relationship between P wave dispersion (Pd) and coronary heart disease complicated with paroxysmal atrial fibrillation (PAF). Methods The maximum duration of Pd and P wave in 112 patients with coronary heart disease complicated with PAF (observation group) were observed and compared with Pd and P max in 84 patients without PAF (control group) . Results The Pd and P max values in the observation group were significantly different from those in the control group (P <0.01). Pd or Pd + P max had a higher specificity for patients with coronary heart disease complicated with paroxysmal atrial fibrillation (82.1 %, 90.5%) and positive predictive value (86.4%, 91.5%). Conclusion Pd is a reliable indicator of surface ECG in patients with coronary heart disease complicated by paroxysmal atrial fibrillation.
