目的 探讨皮肤和皮下脂肪立“8”字缝合对预防腹部手术下腹部纵切口因脂肪液化而非感染性部分裂开的效果。方法 对皮下脂肪组织厚达 5cm以上的切口 98例 ,随机分为观察组 5 0例 ,采用立“8”字缝合皮肤和皮下组织 ;对照组 4 8例 ,采用传统缝合方法。结果 观察组切口一期愈合率 96 0 0 % ,显著高于对照组的79 17% (P <0 0 1) ;脂肪液化切口裂开 4 0 0 % ,显著低于对照组 2 0 83 % (P <0 0 5 ) ,术后平均住院日显著少于对照组 (P <0 0 1)。切口部分裂开的 12例中 7例采用立“8”字二期缝合 ,其愈合率达 10 0 0 0 %。结论 脂肪组织厚达 5cm以上的切口 ,采用立“8”字缝合皮肤和皮下脂肪对预防腹部手术下腹部纵切口脂肪液化切口裂开是一种简便易行、疗效可靠的方法。
Objective To investigate the effect of suturing “8” sutures of skin and subcutaneous fat on the prevention of partial liquefaction of adipose and non-infectious parts under abdominal surgery. Methods 98 cases of subcutaneous fat tissue more than 5cm in thickness were randomly divided into observation group (50 cases) and sutured skin and subcutaneous tissue (8 cases). In the control group, 48 cases were treated by traditional method. Results The rate of primary incision healing in the observation group was 96 0 0%, which was significantly higher than that in the control group 79 17% (P 0 01). The fatty liquefied incision 40 000% was significantly lower than the control group 20 83% P <0 05). The average postoperative hospital stay was significantly less than that of the control group (P <0.01). Seven of the 12 cases with incision were divided into two parts by using “8” suture. The healing rate was 100%. Conclusion It is an easy and effective method to prevent incision of fatty liquefied incision in the longitudinal incision of the abdomen under abdominal operation by suturing the skin and subcutaneous fat with “8” sutures.
