目的:探讨利用Tei指数评价妊娠晚期妊娠期高血压疾病孕妇左心室功能及胎儿心功能的价值。方法:应用超声心动图检测106例妊娠高血压孕妇(分重度子痫前期、轻度子痫前期、妊娠高血压3组)及40例与其孕龄匹配的血压正常孕妇左房前后径(LAD)、左室舒张末期内径(LVD)、左室射血分数(EF)、二尖瓣口频谱E峰流速(VE)、A峰流速(VA)、E/A比值、等容舒张时间(IRT)、等容收缩时间(ICT)、和左室射血时间(ET),计算Tei指数。再用同样的方法测定胎儿左、右心室Tei指数。结果:妊娠期高血压疾病左房结构改变及左室功能减低者胎儿左、右心室Tei指数均显著高于正常胎儿组,差异有统计学意义(P<0.05)。结论:重度子痫前期不仅会影响母体心脏功能,也能累及损伤胎儿心脏功能,Tei指数是一种简单可靠的综合评价母体及胎儿心功能受累的指标。
Objective: To explore the value of using Tei index to evaluate left ventricular function and fetal heart function in pregnant women with gestational hypertension during pregnancy. Methods: Echocardiography was used to detect the left atrium diameter (LAD) of 106 pregnant women with gestational hypertension (severe preeclampsia, mild preeclampsia and gestational hypertension) and 40 normal pregnant women with normotensive pregnancy. (LVD), left ventricular ejection fraction (EVE), mitral E peak velocity (VE), A peak velocity (VA), E / A ratio and isovolumic relaxation time (IRT) , Isovolumic contractile time (ICT), and left ventricular ejection time (ET), calculated Tei index. Then use the same method to determine fetal left and right ventricular Tei index. Results: The left ventricular and left ventricular Tei index of hypertensive disorder complicating pregnancy were significantly higher than those of normal fetus (P <0.05). Conclusion: Severe preeclampsia not only affects maternal cardiac function but also impairs fetal cardiac function. Tei index is a simple and reliable index for comprehensive evaluation of maternal and fetal cardiac function.
