目的分析和评价天津市和平区全球基金流动人口结核病项目实施效果,为探索有效的流动人口结核病防治模式提供科学依据。方法发挥全球基金项目资源优势、采取综合措施开展流动人口结核病控制工作,对项目实施期间天津市和平区流动人口肺结核病人发现和治疗管理情况进行分析。结果项目实施期间天津市和平区共登记流动人口肺结核患者464例,男性296例(63.8%),女性168例(36.2%),男女性别比为1.8:1,平均年龄为(30.00±11.37)岁。患者来自全国24个省市,其中以河北省94例(20.3%),山东54例(11.2%)居多。患者构成以初治涂阴为主,占71.6%(332/464),新涂阳占22.2%(103/464)。患者的来源以转诊为主,占84.9%(394/464)。涂阳患者2月末痰菌阴转率为91.7%(121/132),新涂阳肺结核患者成功治疗率为93.2%(96/103),全部活动性肺结核患者成功治疗率为92.5%(429/464)。结论全球基金项目在流动人口结核病控制工作中效果显著,为制定有效的流动人口结核病防治策略积累宝贵经验。
Objective To analyze and evaluate the implementation effect of the Global Fund Floating Population TB Project in Heping District of Tianjin and provide a scientific basis for exploring effective prevention and treatment of tuberculosis in floating population. Methods Taking advantage of the resources of global fund projects, the Bank took comprehensive measures to carry out TB control in floating population and analyzed the management and discovery of tuberculosis cases among floating population in Heping District of Tianjin during the project implementation. Results During the implementation of the project, a total of 464 floating-population TB patients were registered in the Heping District of Tianjin Municipality, 296 (63.8%) were male and 168 (36.2%) were female, with a sex ratio of 1.8: 1 for men and women and an average age of (30.00 ± 11.37) years . Patients from 24 provinces and cities nationwide, of which 94 cases in Hebei Province (20.3%), Shandong Province, 54 cases (11.2%). The patients were primarily smear-negative, accounting for 71.6% (332/464) and newly smear-positive smear accounting for 22.2% (103/464). The source of the patients was referral, accounting for 84.9% (394/464). In smear-positive patients, sputum negative conversion rate was 91.7% (121/132) at the end of February, successful treatment rate was 93.2% (96/103) in newly smear-positive pulmonary tuberculosis patients, and successful treatment rate in all active tuberculosis patients was 92.5% (429 / 464). Conclusion The Global Fund Project has achieved remarkable results in the control of TB in floating population and accumulated valuable experience in developing an effective prevention and control strategy for floating population.
