目的通过观察2型糖尿病伴肥胖与非肥胖患者的代谢状态,了解2型糖尿病伴肥胖患者代谢紊乱的特点。方法将102例2型糖尿病患者按体重指数(BM I)分为肥胖组55例(BM I≥25kg/m2)和非肥胖组47例(BM I<25kg/m2)。测定血压(SBP/DBP)、空腹血糖(FPG)、糖化血红蛋白(GHbA1 c)、血浆空腹胰岛素(F-ins)、血总胆固醇(TC)、甘油三酯(TG)、高密度脂蛋白胆固醇(HDL-C)、低密度脂蛋白胆固醇(LDL-C)、24h尿白蛋白排泄率(UAER)、血尿酸(BUA)。结果肥胖组SBP、TG、LDL-C、F-ins、UAER、BUA明显高于非肥胖组,HDL-C较非肥胖组明显低,两组间比较差异均有显著性;TC、FBG、GHbA1 c水平两组间差异无显著性。结论2型糖尿病伴肥胖患者有明显的代谢紊乱。BM I是影响糖尿病患者脂代谢、血压、胰岛素水平、血尿酸水平及尿白蛋白排泄率的重要因素。对2型糖尿病伴有肥胖患者应予以重视,对其代谢异常进行综合治疗。
Objective To observe the metabolic status of type 2 diabetes mellitus patients with obesity and non-obesity to understand the characteristics of metabolic disorders in type 2 diabetes patients with obesity. Methods Totally 102 patients with type 2 diabetes were divided into body mass index (BMI): obesity group (BM I> 25kg / m2) and obesity group (BMI> 25kg / m2). Blood pressure (SBP / DBP), fasting plasma glucose (FPG), glycosylated hemoglobin (GHbA1 c), plasma fasting insulin (F-ins), total cholesterol (TC), triglyceride (TG), high density lipoprotein cholesterol HDL-C), low density lipoprotein cholesterol (LDL-C), urinary albumin excretion rate (UAER) and serum creatinine (BUA). Results The levels of SBP, TG, LDL-C, F-ins, UAER and BUA in obese group were significantly higher than those in non-obese group and HDL-C was significantly lower than those in non-obese group c level between the two groups was no significant difference. Conclusion Type 2 diabetes patients with obesity have obvious metabolic disorders. BM I is an important factor influencing lipid metabolism, blood pressure, insulin level, serum uric acid level and urinary albumin excretion rate in diabetic patients. Patients with type 2 diabetes associated with obesity should pay attention to its comprehensive treatment of metabolic abnormalities.
