目的评估预防接种“妈妈课堂”对儿童接种第二类疫苗的影响。方法于2015年9—12月采用二阶段整群抽样方法抽取嘉兴市南湖区16所公立幼儿园32个小班儿童,对其主要监护人进行问卷调查,按是否参加过“妈妈课堂”分成两组,比较第二类疫苗知识知晓率和儿童第二类疫苗接种率,并分析第二类疫苗接种的影响因素。结果有效调查758名主要监护人,“妈妈课堂”参加率为24.80%(188/758)。参加组对第二类疫苗是否自费、可预防疾病和狂犬病疫苗接种适应证的知晓率分别为96.81%、29.79%和51.60%,均高于未参加组的90.00%、16.49%和42.63%(P<0.05)。参加组儿童b型流行性感冒嗜血杆菌多糖结合疫苗(Hib)、口服轮状病毒减毒活疫苗(ORV)和七价肺炎球菌结合疫苗(PCV7)的接种率分别为73.40%、57.45%和43.09%,均高于未参加组的61.23%、48.42%和29.30%(P<0.05)。多因素Logistic回归分析结果显示,参加“妈妈课堂”是接种Hib(OR=1.958,95%CI:1.315~2.913)、ORV(OR=1.709,95%CI:1.169~2.497)和PCV7(OR=1.821,95%CI:1.276~2.597)的促进因素。结论 “妈妈课堂”可提高儿童主要监护人对第二类疫苗的正确认知,促进第二类疫苗的接种。
Objectives To assess the impact of vaccination “mom class ” on the second vaccination of children. Methods A total of 32 small class children from 16 public kindergartens in Nanhu District of Jiaxing City were collected by two-stage cluster sampling method from September to December in 2015. The main guardians were surveyed by questionnaire, divided into two groups according to whether they participated in “MOM Classroom” , Comparing the knowledge of the second type of vaccine and the second type of vaccination rate in children, and analyzing the influencing factors of the second type of vaccination. Results Effective survey 758 main guardians, “mom class ” participation rate was 24.80% (188/758). Participants’ awareness of whether the second type of vaccine was self-funded, preventable disease and rabies vaccination indications were 96.81%, 29.79% and 51.60%, respectively, higher than 90.00%, 16.49% and 42.63% of the non-participating group <0.05). The inoculation rates of Hib, HAV and ORV and PCV7 were 73.40%, 57.45% and 43.09%, higher than 61.23%, 48.42% and 29.30% of the non-participating group (P <0.05). Multivariate Logistic regression analysis showed that Hib (OR = 1.958, 95% CI: 1.315-2.913), ORV (OR = 1.709, 95% CI: 1.169-2.497) and PCV7 = 1.821, 95% CI: 1.276 ~ 2.597). Conclusion “MOM Classroom ” can raise the primary guardian of children’s correct cognition of the second type of vaccine and promote the vaccination of the second type of vaccine.
