目的探讨血清色素上皮源性因子(sPEDF)与2型糖尿病(T2DM)胰岛素抵抗的关系。方法采用酶联免疫法检测176例受试者[糖调节正常组(NGT)52例,T2DM 124例]的sPEDF水平,同时检测空腹C肽(FCP)、2小时C肽(2h CP)糖化血红蛋白(HbA1c)、血糖、血脂、高敏C反应蛋白(hs-CRP)等生化指标的水平。结果 (1)T2DM组sPEDF水平显著高于NGT组[2893.45(1641.91~5533.13)μg/L vs 2672.95(1486.54~4039.13)μg/L(P<0.05)];(2)按sPEDF水平四分位分组,上四分位点的胰岛素抵抗指数(HO-MA-IR)值显著高于下四分位点者[1.90(1.60~2.70)vs 1.45(0.90~2.10)(P<0.01)];(3)单变量相关关系分析显示:sPEDF水平与HOMA-IR、SBP、DBP、FCP、hs-CRP呈显著正相关(r=0.206,0.210,0.170,0.213,0.221,P<0.05或0.01);(4)逐步多元回归显示HOMA-IR(β=0.174,P<0.05),SBP(β=0.189,P<0.05)是sPEDF的独立影响因素。结论中国人群中2型糖尿病患者sPEDF水平显著升高,与胰岛素抵抗指数显著独立相关。
Objective To investigate the relationship between serum pigment epithelium-derived factor (sPEDF) and type 2 diabetes mellitus (T2DM) insulin resistance. Methods The serum levels of sPEDF in 176 subjects (52 normal glucose control group and 124 T2DM patients) were detected by enzyme-linked immunosorbent assay (ELISA), and fasting C-peptide (FCP), 2-hour C peptide (HbA1c), blood glucose, blood lipids, high-sensitivity C-reactive protein (hs-CRP) and other biochemical indicators. Results (1) The level of sPEDF in T2DM group was significantly higher than that in NGT group [2893.45 (1641.91 ~ 5533.13 μg / L vs 2672.95 [1486.54 ~ 4039.13] μg / L] (P <0.05) (1.90 (1.60-2.70) vs 1.45 (0.90-2.10) (P <0.01)]; (3) The HO-MA-IR in the upper quartile was significantly higher than that in the lower quartile ) Univariate correlation analysis showed that sPEDF level was positively correlated with HOMA-IR, SBP, DBP, FCP and hs-CRP (r = 0.206,0.210,0.170,0.213,0.221, P <0.05 or 0.01) ) Stepwise multiple regression showed that HOMA-IR (β = 0.174, P <0.05) and SBP (β = 0.189, P <0.05) were the independent influencing factors of sPEDF. Conclusions The level of sPEDF in type 2 diabetes mellitus patients in China is significantly increased, which is independent of insulin resistance index.
