目的 探讨胃间质瘤 (GST)中p1 6、p53、c myc基因蛋白及增殖细胞核抗原 (PCNA)与预后的关系。方法 应用免疫组化S P方法对 71例GST组织进行p1 6、p53、c myc基因蛋白和PCNA检测 ,并将PCNA的不同表达进行细胞增殖指数 (PI)分级。结果 71例GST中p1 6、p53、c myc基因蛋白阳性表达率分别为 :恶性 34 .8%、95 .7%、1 0 0 % ;交界性 45 .5 %、77.3 %、90 .9% ;良性 65 .4%、73 .1 %、65 .4%。p1 6蛋白表达PI分级Ⅰ、Ⅱ级明显高于Ⅲ、Ⅳ级(P <0 .0 1 ) ,存活期 <3年的明显低于 >3年的 (P <0 .0 1 )。p1 6蛋白随肿瘤分化程度和生存率降低 ,阳性表达率也逐渐降低 ;而p53、c myc蛋白及PI分级则相反。p1 6与p53、c myc蛋白及PCNA的PI分级有明显负相关性(P <0 .0 5)。结论 p1 6、p53、c myc基因蛋白和PCNA的PI分级有助于判断GST的恶性程度和预测预后
Objective To investigate the relationship between prognosis and prognosis of p16, p53, c-myc gene and proliferating cell nuclear antigen (PCNA) in gastric stromal tumors (GST). Methods Immunohistochemical SP method was used to detect the expression of p16, p53, c-myc and PCNA in 71 cases of GST tissues, and the different expression of PCNA was graded for cell proliferation index (PI). Results The positive rates of p16, p53 and c-myc in 71 cases of GST were: malignant 34.8%, 95.7%, 100%; borderline 45.5%, 77.3%, 90.9% ; Benign 65.4%, 73.1%, 65.4%. The expression of p16 protein in PI grade Ⅰ and Ⅱ was significantly higher than those in grade Ⅲ and Ⅳ (P <0.01), and the survival time was significantly lower than 3 years (P <0.01). The expression of p16 protein decreased with the degree of tumor differentiation and the survival rate. The positive rate of p16 protein also decreased gradually. However, the expression of p53, c-myc and PI was opposite. There was a significant negative correlation between p1 6 and p53, c-myc protein and PCNA PI grade (P <0.05). Conclusions PI classification of p16, p53, c-myc and PCNA may help to determine the degree of malignancy and prognosis of GST
