目的探讨铅作业人群细胞色素氧化酶P450酶系中的CYP2D6基因拷贝数多态性与铅毒性引起的血铅、尿铅水平的关系。方法选择广州市某蓄电池行业233名铅作业工人,按铅尘接触水平分为高浓度组和低浓度组2组,采用统一调查表进行职业性健康问卷调查;采用石墨炉原子吸收光谱法测定现场空气铅尘及铅作业工人血铅、尿铅水平;提取铅作业人群周围血DNA,采用实时荧光定量聚合酶链式反应(PCR)法和ΔCt值法检测铅作业工人的CYP2D6基因拷贝数,分析不同铅尘作业环境下作业人员CYP2D6基因拷贝数多态性与血铅、尿铅水平的关系。结果铅作业工人CYP2D6基因拷贝数与血铅、尿铅水平有关,2组中拷贝数小于2的作业人员血铅浓度均高于拷贝数为2的人群(P﹤0.001),低浓度组拷贝数小于2的研究对象尿铅浓度比拷贝数为2的人群高(P=0.03),尚未发现年龄、性别、工龄、学历与血铅水平和尿铅水平的关系差异有统计学意义。结论 CYP2D6基因拷贝数多态性与铅毒性引起的血铅尿铅水平有关,拷贝数少于2的CYP2D6基因可能是铅中毒易感基因型。
Objective To investigate the relationship between CYP2D6 gene copy number polymorphism in lead-cytochrome P450 enzymes and blood lead and urine lead levels caused by lead toxicity. Methods A total of 233 lead workers in a battery industry in Guangzhou were divided into two groups according to exposure level of lead and dust: Occupational Health Questionnaire (QHSR) was conducted by using a unified questionnaire, and the site was determined by graphite furnace atomic absorption spectrometry Air lead dust and lead workers blood lead and urine lead levels; extraction of peripheral blood DNA of lead workers, using real-time fluorescence quantitative polymerase chain reaction (PCR) and ΔCt value detection of lead workers CYP2D6 gene copy number analysis Relationship between CYP2D6 Gene Copy Number Polymorphism and Blood Lead and Urinary Lead in Workers with Different Lead Dust Operating Conditions. Results The copy number of CYP2D6 gene in lead workers was related to blood lead and urine lead levels. Blood lead levels of workers with copy number less than 2 were higher than those with copy number 2 (P <0.001) Urine levels were lower in subjects than in those with a copy number of 2 (P = .03). No significant association was found between age, sex, length of service, education and blood lead levels, and urine lead levels. Conclusion CYP2D6 gene copy number polymorphism is related to the level of blood lead and lead caused by lead toxicity. CYP2D6 gene with copy number less than 2 may be the susceptible genotype of lead poisoning.
